摘要 目的研究早期母婴皮肤接触对新生儿喂养不耐受的影响分析。方法选取2018年3月至2019年12月苏州市第九人民医院收治的60例喂养不耐受新生儿为研究对象,随机数字表法分为对照组和观察组,各组30例;对照组采用常规治疗,观察组在常规治疗的基础上采用早期母婴皮肤接触,比较两组的胃管留置时间、每日睡眠时间和住院时间,生长发育情况、消化道不良反应的发生情况及神经行为发育。结果观察组胃管留置时间、每日睡眠时间和住院时间分别为(3.52±1.26)d、(18.03±1.96)h、(12.59±3.46)d,对照组分别为(4.39±1.34)d、(16.42±1.35)h、(15.18±4.12)d;观察组胃管留置时间和住院时间低于对照组,观察组每日睡眠时间高于对照组(P<0.05)。治疗前观察组体质量、头围、身长分别为(3220.57±99.23)g、(34.59±1.25)cm、(50.93±1.53)cm,对照组分别为(3216.49±98.52)g、(34.57±1.29)cm、(50.87±1.59)cm;治疗前两组新生儿体质量、头围和身长差异无统计学意义(P>0.05);治疗后观察组体质量、头围和身长分别为(3369.56±102.69)g、(35.89±1.92)cm、(53.63±2.49)cm,对照组分别为(3294.43±100.63)g、(35.13±1.68)cm、(52.09±2.17)cm;治疗后观察组体质量、头围和身长高于对照组(P<0.05)。观察组不良反应发生率为23.33%(7/30),对照组为60.00%(18/30);观察组不良反应发生率低于对照组(P<0.05)。观察组一般反应、原始反射、主动肌张力、被动肌张力、行为能力和总分分别为(5.82±0.36)分、(5.93±0.56)分、(7.26±0.85)分、(7.29±0.76)分、(11.52±0.85)分、(38.85±1.24)分,对照组分别为(5.90±0.37)分、(5.62±0.41)分、(6.52±0.78)分、(6.68±0.64)分、(10.43±0.69)分、(36.67±1.07)分,观察组原始反射、主动肌张力、被动肌张力、行为能力和总分高于对照组(P<0.05);两组一般反应比较差异无统计学意义(P>0.05)。结论采用早期母婴皮肤接触对改善新生儿喂养不耐受的效果较好,缩短胃管留置时间,改善睡眠情况,促进新生儿生长发育和神经行为发育。 Objective To study the effect of early mother-to-child skin contact on neonatal feeding intolerance.Methods Sixty infants with intolerance to feeding from March 2018 to December 2019 from the Ninth People's Hospital of Suzhou were taken as the research objects according to random number table,and were divided into a control group and an observation group,with 30 cases in each group;the control group was treated with conventional treatment,and the observation group was based on conventional treatment.Early skin-to-mother contact was used to compare the gastric tube indwelling time,daily sleep time,and hospitalization time,growth and development,occurrence of gastrointestinal adverse reactions,and neurobehavioral development in the two groups.Results The gastric tube indwelling time,daily sleep time and hospital stay in the observation group were(3.52±1.26)d,(18.03±1.96)h,(12.59±3.46)d,and the control group were(4.39±1.34)d,(16.42±1.35)h,(15.18±4.12)d;The gastric tube indwelling time and hospital stay time in the observation group were lower than those in the control group,and the daily sleep time in the observation group was higher than that in the control group(P<0.05).Before treatment,the body weight,head circumference,and length of the observation group were(3220.57±99.23)g,(34.59±1.25)cm,and(50.93±1.53)cm,and the control group were(3216.49±98.52)g and(34.57±1.29),respectively.cm,(50.87±1.59)cm;there was no significant difference in body weight,head circumference,and length of the two groups of newborns before treatment(P>0.05);the body weight,head circumference,and length of the observation group after treatment were(3369.56±102.69)g,(35.89±1.92)cm,(53.63±2.49)cm,the control group was(3294.43±100.63)g,(35.13±1.68)cm,(52.09±2.17)cm;the group body was observed after treatment The quality,head circumference and length were higher than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was 23.33%(7/30)and 60.00%(18/30)in the control group;the incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).The general response,original reflex,active muscle tone,passive muscle tone,behavioral capacity,and total score of the observation group were(5.82±0.36)points,(5.93±0.56)points,(7.26±0.85)points,(7.29±0.76)points(11.52±0.85)points,(38.85±1.24)points,and the control group were(5.90±0.37)points,(5.62±0.41)points,(6.52±0.78)points,(6.68±0.64)points,and(10.43±0.69)points and(36.67±1.07)points.The observation group's original reflexes,active muscle tone,passive muscle tone,behavioral ability,and total score were higher than those of the control group(P<0.05).There was no significant difference in general response between the two groups(P>0.05).Conclusion Early mother-to-infant skin contact has a better effect on neonatal feeding intolerance,shortens the period of gastric tube indwelling,improves sleep,and promotes neonatal growth and neurobehavioral development.
