经皮微创内固定术治疗四肢粉碎性骨折的临床应用效果观察

Clinical Observation of Minimally Invasive Percutaneous Internal Fixation in the Treatment of Comminuted Limb Fractures

目的探讨对四肢粉碎性骨折患者施行经皮微创内固定术法治疗的应用价值。方法将苏州市吴中区甪直人民医院2018年1月至2020年1月收治的四肢粉碎性骨折120例患者作为研究对象,按照随机自愿原则分为研究组(60例)与对照组(60例)。研究组采取经皮微创内固定术,对照组采取传统钢板固定术。对比两组患者的临床手术指标、术后血清炎症因子、临床疗效及并发症发生情况等。结果在手术指标方面,研究组患者术中出血量、手术时间、伤口愈合时间、术后疼痛评分等指标均优于对照组,组间比较差异有统计学意义(P<0.05);在术后血清炎症因子方面,研究组患者降钙素原、术后1~3天的C反应蛋白等指标均低于对照组,组间比较差异有统计学意义(P<0.05);在临床疗效方面,研究组指标显著优于对照组,组间比较差异有统计学意义(P<0.05);在并发症出现状况方面,研究组显著低于对照组,组间比较差异有统计学意义(P<0.05)。结论四肢粉碎性骨折患者行经皮微创内固定术治疗,可以降低患者的术后炎症反应和并发症的出现状况,减轻患者的术后痛苦,缩短患者手术时间,帮助患者进一步恢复,使患者的术后生活质量显著提升。

Objective To explore the application value of minimally invasive percutaneous internal fixation for patients with comminuted fracture of limbs.Methods From January 2018 to January 2020,120 patients with comminuted fractures of limbs admitted to Luzhi People's Hospital of Wuzhong District,Suzhou City were selected as the research objects,and were randomly divided into two groups:the research group and the control group,with 60 cases in each group.The study group received minimally invasive percutaneous internal fixation,while the control group received traditional plate fixation.The clinical operation indexes,postoperative serum inflammatory factors,clinical efficacy and complications of the two groups were compared.Results In terms of operation indexes,the indexes of blood loss,operation time,wound healing time and postoperative pain score in the study group were better than those in the control group,and the difference between the two groups was statistically significant(P<0.05).In the aspect of serum inflammatory factors after operation,the indexes of procalcitonin and C-reactive protein in 1~3 days after operation in the study group were lower than those in the control group,and the difference between the two groups was statistically significant(P<0.05).In terms of clinical efficacy,the indexes of the study group were significantly better than those of the control group,and the difference between the two groups was statistically significant(P<0.05).The incidence of complications in the study group was significantly lower than that in the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusion Percutaneous minimally invasive internal fixation for patients with comminuted fracture of limbs can reduce postoperative inflammatory reaction and complications,relieve postoperative pain,shorten operation time,help patients recover further,and significantly improve their postoperative quality of life.
