后腹腔镜下肾部分切除术治疗肾肿瘤的疗效分析

Analysis of the Curative Effect of Partial Nephrectomy under Retro Peritoneal Laparoscopic Surgery for Renal Tumors

目的研究后腹腔镜下肾部分切除术治疗肾肿瘤的疗效。方法选取2018年1月至2019年1月青岛慧康医院收治的108例肾肿瘤患者,按照数字表法随机分为对照组和观察组,各54例。对照组采用传统开放性肾部分切除术治疗,观察组采用后腹腔镜下肾部分切除术治疗。比较两组患者的术中情况、术后恢复情况、并发症发生情况。结果两组患者手术时间比较,差异无统计学意义(P>0.05)。观察组肾动脉阻断时间(18.35±1.04)min短于对照组的(30.14±1.65)min,观察组术中出血量(99.71±3.65)mL少于对照组的(275.45±3.18)mL,差异有统计学意义(P<0.05)。观察组术后肛门排气时间(2.04±0.16)d、术后置管时间(2.96±0.34)d、平均住院时间(10.73±0.54)d,分别短于对照组的(3.99±0.42)d、(5.96±0.90)d、(15.13±1.25)d,差异有统计学意义(P<0.05)。结论在肾肿瘤的治疗中,采用后腹腔镜下肾部分切除术治疗,能够有效缩短肾动脉阻断时间,减少术中出血量,同时可加快术后恢复速度,降低术后并发症发生率,临床疗效更为理想。

Objective To study the effect of retroperitoneal partial nephrectomy in the treatment of renal tumors.Methods A total of 108 patients with renal tumor admitted to Qingdao Huikang Hospital from January 2018 to January 2019 were randomly divided into control group and observation group,54 cases in each group.The control group was treated with traditional open partial nephrectomy,and the observation group was treated with retroperitoneal laparoscopic partial nephrectomy.The intraoperative condition,postoperative recovery and complications of the two groups were compared.Results There was no significant difference in the operation time between the two groups(P>0.05);the renal artery block time of the observation group was(18.35±1.04)min shorter than that of the control group(30.14±1.65)min,and the intraoperative blood loss in the observation group(99.71±3.65)mL is less than the control group(275.45±3.18)mL,there is a significant difference(P<0.05).The postoperative anal exhaust time(2.04±0.16)d,postoperative catheterization time(2.96±0.34)d,and average hospital stay(10.73±0.54)d in the observation group were shorter than those in the control group(3.99±0.42)d,(5.96±0.90)d,(15.13±1.25)d,there was a significant difference(P<0.05).Conclusion In the treatment of renal tumors,retroperitoneal laparoscopic partial nephrectomy can effectively shorten the time of renal artery occlusion,reduce intraoperative blood loss,speed up postoperative recovery,and reduce postoperative complications.The clinical efficacy is more ideal.
