芜湖地区新生儿及婴幼儿TORCH感染的血清学检查分析 被引量:5

摘要 目的了解芜湖地区新生儿及婴幼儿TORCH感染情况和感染特点。方法选择2018年11月—2019年11月本院门诊和住院的年龄为0~3岁的新生儿及婴幼儿共1 091例作为研究对象,应用电化学发光法(CLIA)检测新生儿和婴幼儿血清中的TORCH特异性抗体,统计分析血清TOX-IgM、TOX-IgG、RV-IgM、RV-IgG、CMV-IgM、CMV-IgG、HSVⅠ-IgG及HSVⅡ-IgG的阳性率。结果受检新生儿及婴幼儿TORCH-IgM总阳性率为0.92%,其中以CMV感染为主(0.64%);TORCH-IgG总阳性率为99.82%,也以CMV感染最为常见(98.08%)。在TORCH-IgM阳性率中,新生儿发现1例TOX-IgM感染、1例RV-IgM感染;而婴儿以CMV-IgM感染最高(7例),RV-IgM感染1例;幼儿发现1例TOX-IgM感染。在TORCH-IgG阳性率中,新生儿以CMV-IgG感染率最高(98.17%),婴儿也以CMV-IgG感染率最高(100.00%),而幼儿以RV-IgG感染率最高(100.00%)。不同性别分析发现,男女性新生儿和婴幼儿TORCH-IgM、TORCH-IgG总阳性率比较差异无统计学意义(P>0.05)。混合感染模式分析发现,CMV与HSV特异性抗体IgG同时阳性的比例最高(79.56%)。结论芜湖地区TORCH感染主要以CMV和HSV感染及二者混合感染为主,既往感染率较高,急性感染率较低,临床上应加强孕妇和新生儿及婴幼儿TORCH检测筛查,为相关疾病的诊断治疗提供相关依据。 Objective To understand the status and characteristics of TORCH infection in newborns,infants and young children in Wuhu.Methods A total of 1091 newborns,infants and young children(the age of 0~3) inpatients or outpatients from our hospital were choose as the research object during November 2018 to November 2019.Electrochemiluminescence(CLIA) was used to detect TORCH specific antibodies,the positive rate of TOX-IgM,TOX-IgG,RV-IgM,RV-IgG,CMV-IgM,CMV-IgG,HSV-IgM and HSV-IgG in serum of all the cases were analyzed.Results The total positive rate of TORCH-IgM in newborns,infants and young children was 0.92%,of which CMV infection was the main(0.64%);the total positive rate of TORCH-IgG was 99.82%,of which CMV was the most common(98.08%) infection.Among the positive cases of TORCH-IgM,1 case of TOX-IgM infection and 1 case of RV-IgM infection were found in newborns.CMV-IgM infection(7 cases) was the highest in infants,among which there were one case of RV-IgM infection,and one case of TOX-IgM infection was found in young children.Among the TORCH-IgGpositive cases,CMV-IgG accounting for the highest in both newborns(98.17%) and the infants(100.00%),while young children had the highest infection rate of RV-IgG(100.00%).Between male and female,there was no significant difference in the total positive rates of TORCH-IgM and TORCH-IgG in newborns,infants and young children(P>0.05).Double positive of CMV and HSV specific antibody IgG account for the highest proportion(79.56%) according to the analysis of mixed.Conclusions TORCH infection in Wuhu area is mainly caused by CMV and HSV infections and their mixed infections.The previous infection rate is high and the acute infection rate is low.Clinically,TORCH detection and screening among pregnant women,newborns and infants should be strengthened to provide relevant evidence for diagnosis and treatment.
