6547例妇科门诊患者阴道分泌物检测结果分析

Analysis of the detection results of vaginal secretions in 6547 gynecological outpatients

目的分析本院6 547例妇科门诊患者阴道分泌物清洁度、细菌性、滴虫性和真菌性阴道病的检测情况。方法阴道分泌物进行常规镜检和细菌性阴道病(BV)四联检测,结果按年龄分5组进行清洁度、BV、滴虫和真菌的感染率比较。结果 6 547例阴道分泌物清洁度Ⅰ~Ⅳ分别占3.0%、37.3%、42.2%和17.5%;清洁度正常和异常分别占40.3%和59.7%。25岁~34岁年龄组清洁度Ⅲ度和>55岁年龄组清洁度Ⅳ度检出率最高,这2组清洁度异常发生率明显高于其他各年龄组(P<0.01)。BV、滴虫和真菌的检出率分别为14.5%、3.5%和17.2%。随着清洁度等级上升,三类阴道病的检出率逐级升高。25岁~34岁和35岁~44岁2个年龄组BV和真菌检出率明显高于其余各组(P<0.01),滴虫检出率的年龄分布特征则呈现两头低中间高的特点。阴道清洁度异常患者BV、滴虫和真菌的检出率均明显高于清洁度正常患者(P<0.01)。结论本院妇科患者真菌性和细菌性阴道病发病率较高,3种阴道病的阳性率与阴道分泌物清洁度及患者年龄有密切关系。

Objective The cleanliness,bacteriosis,trichomoniasis and fungal vaginosis of vaginal secretions in 6 547 gynecological outpatients in our hospital were analyzed.Methods Routine microscopic examination and bacterial vaginosis( BV)quadruple detection were carried out for vaginal secretions.The results were divided into 5 groups according to age to compare the infection rates of cleanliness,BV,trichomonas and fungi.Results In 6 547 vaginal secretion,vaginal cleanliness Ⅰ to Ⅳwere 3.0%,37.3%,42.2% and 17.5% respectively.The cleanliness was 40.3%( normal) and 59.7%( abnormal).Cleanliness Ⅲ in 25-35 age group and cleanliness Ⅳ in>55 age group degree is the highest,and the rates of cleanliness abnormality in these two groups were significantly higher than that in other age groups.BV,trichomonas,fungi,three kinds of vaginosis rates were 14.5%,3.5% and 17.2%.Positive rate of three vaginosis increases with cleanliness level increases.The detection rates of BV and fungi in 25-34 and 35-44 two age groups were significantly higher than those in other age groups( P<0.01).The age characteristics of the detection rate of trichomonas are low at both ends and high in the middle.The detection rates of BV,trichomonas and fungi in cleanliness abnormal patients were significantly higher than that of cleanliness normal patients( P<0.01).Conclusion Fungi and bacterial vaginosis of gynecological patients in our hospital have higher prevalence,and the positive rate of the three vaginosis is closely related to the cleanliness of vaginal discharge and the age of patients.
