中老年全膝关节置换术后患者延续性护理需求及影响因素研究

Investigation on the continuous nursing needs and influencing factors of middle aged and older patients after total knee arthroplasty

目的了解中老年全膝关节置换术后患者的延续性护理需求现状,并分析其影响因素。方法采用便利抽样法,对356例全膝关节置换术后患者,采用一般资料调查表、中老年全膝关节置换术患者延续护理需求量表和简明健康调查量表中文版进行调查。结果中老年全膝关节置换术后患者延续性护理服务需求总分为(110.47±7.08)分,需求程度较高,其中生理需求维度得分最高,自我实现需求维度得分最低;生活质量总分为(374.51±88.52)分。68.5%的患者希望通过网络沟通平台提供延续护理服务,66.0%的患者希望电话随访。多元线性回归分析结果显示,生活质量、年龄是中老年全膝关节置换术后患者延续护理服务需求的主要影响因素(R2=0.444,F=142.943,P<0.01)。结论中老年全膝关节置换术后患者延续性护理需求较高,应根据患者的需求、年龄及生活质量水平制订科学有效的、个体化延续护理干预措施,促进患者康复。

Objective To understand the status of continuous nursing needs of middle aged and older patients after total knee arthroplasty in Northern Henan province and analyze the influencing factors.Methods A total of 356 patients undergoing total knee arthroplasty were investigated using the general data questionnaire,the continuing care demand scale for middle aged and older patients with total knee arthroplasty,and the Medical Outcomes Study item short from health survey(SF-36).Results The total demand for continuous care services of patients after total knee replacement was(110.47±7.08),with a high degree of demand,in which the physiological needs dimension score was the highest,and the self-actualization demand dimension score was the lowest;the total quality of life score was(374.51±88.52).Continuing care services through the network communication platform were needed by 68.5%of patients,and following up by phone of 66.0%.The multiple linear regression analysis showed that quality of life and age were the main factors influencing the demand for continuous nursing service after total knee replacement(R2=0.444,F=142.943,P<0.01).Conclusion The quality of life of middle aged and older patients after total knee arthroplasty is poor.According to the needs,age and quality of life of patients,scientific and effective,individualized continuous nursing intervention measures should be formulated to promote the rehabilitation of patients.
