加速康复外科理念在腹腔镜袖状胃切除术患者中的运用及效果观察

Application and Effect of Enhanced Recovery after Surgery in Patients with Laparoscopic Sleeve Gastrectomy

目的探讨加速康复外科(enhanced recovery after surgery,ERAS)理念在腹腔镜袖状胃切除术(laparoscopic sleeve gastrectomy,LSG)患者中的运用及临床效果。方法回顾性分析2016年10月~2019年12月在我院接受LSG手术的64例患者,按照围手术期不同处理方式分为ERAS组与对照组,每组各32例。对照组采用传统治疗模式,ERAS组采用快速康复理念进行干预治疗。对两组患者的术后恢复情况和并发症发生率进行比较。结果ERAS组患者较对照组术后肛门首次排气早,疼痛评分低,住院费用少,住院时间短,差异均具有统计学意义(P<0.05)。ERAS组术后呕吐及肺部感染率较对照组低(P<0.05),其他术后并发症无统计学差异(P>0.05)。结论对LSG手术患者围手术期采用快速康复外科理念治疗模式是安全有效的,能够加快患者的术后恢复速度,缩短住院时间,降低住院费用,有效降低术后呕吐及肺部感染并发症发生,具有良好的临床效果,值得进一步推广。

Objective To investigate the application and clinical effect of enhanced recovery surgery(ERAS)in patients with laparoscopic sleeve gastrectomy(LSG).Methods A retrospective analysis of 64 patients undergoing LSG surgery in our hospital from October 2016 to December 2019 were divided into ERAS group and control group according to different perioperative treatment methods,with 32 cases in each group.The control group was treated with conventional treatment mode,and the ERAS group was treated with enhanced recovery mode.The postoperative recovery and complications of the two groups were compared.Results Compared with the control group,patients in the ERAS group had earlier anal exhaust,lower pain scores,less hospitalization costs,and less postoperative hospital stay.The differences were statistically significant(P<0.05).The incidence of postoperative vomiting and pulmonary infection were lower in the ERAS group(P<0.05)without increase of other complications.Conclusion It is safe and effective to adopt ERAS mode for patients with LSG surgery during the perioperative period.It can speed up the postoperative recovery of patients,shorten hospitalization time,reduce the hospitalization costs and reduce the incidence of postoperative vomiting and pulmonary infection.It has good clinical effects and is worthy of further promotion.
