基于基因学高龄产妇产后抑郁的危险因素分析

Analysis of risk factors for postpartum depression in elderly women based on genetics

目的研究高龄产妇产后抑郁现状并基于基因学进行危险因素分析。方法选取2017年5月—2019年1月在河北北方学院附属第一医院妇产科住院分娩的高龄产妇300例为研究对象,进行回顾性研究。统计高龄产妇一般资料,分析产后抑郁发生情况,采用多因素Logistic回归分析法分析产妇产后抑郁的基因学独立危险因素。结果300例高龄产妇中发生产后抑郁87例(29.00%),未发生产后抑郁213例(71.00%);单因素分析结果显示,高龄产妇产后抑郁与产妇脑源性神经营养因子(BDNF)基因rs11030100、rs6265位点及儿茶酚氧甲基转移酶(COMT)基因rs2020917位点的基因型突变有明显相关性(P<0.05)。多因素Logistic回归分析显示,BDNF基因rs11030100位点基因(OR=1.671,95%CI 1.112~2.511)、BDNF基因rs6265位点基因(OR=2.314,95%CI 1.356~3.949)、COMT基因rs2020917位点基因(OR=1.794,95%CI 1.137~2.831)是高龄产妇产后抑郁的独立危险因素(P<0.05)。结论BDNF基因rs11030100、rs6265位点基因及COMT基因rs2020917位点基因是高龄产妇产后抑郁的危险因素,应引起临床足够重视。

Objective To study the current status of postpartum depression in elderly women and analyze risk factors based on genetics.Methods A retrospective study of 300 elderly women who gave birth in the Department of Obstetrics and Gynecology of the First Affiliated Hospital of Hebei North University from May 2017 to January 2019 was selected as the research object.The general data of the elder parturients were collected to analyze the occurrence of postpartum depression,and the multivariate logistic regression analysis method was used to analyze the genetic independent risk factors of postpartum depression.Results Eighty-seven cases of postpartum depression occurred among 300 elderly women(29.00%);univariate analysis showed that postpartum depression in elderly women was associated with maternal brain-derived neurotrophic factor(BDNF)gene rs11030100,rs6265 locus and catecholoxymethyl transfer The genotype mutation at the rs2020917 locus of the COMT gene was significantly correlated(P<0.05).Multivariate logistic regression analysis showed that BDNF gene rs11030100 locus gene(OR=1.671,95%CI 1.112~2.511),BDNF gene rs6265 locus gene(OR=2.314,95%CI 1.356~3.949),COMT gene rs2020917 locus Gene(OR=1.794,95%CI 1.137~2.831)is an independent risk factor for postpartum depression in elderly parturients(P<0.05).Conclusion BDNF gene rs11030100,rs6265 locus gene and COMT gene rs2020917 locus gene are risk factors for postpartum depression in elderly parturients,which should be paid enough clinical attention.
