牙列缺损患者微创环切技术口腔种植的效果分析 被引量:8

Effects of minimally invasive circumcision on oral implantation of patients with dentition defects

摘要 目的:探讨微创环切技术对牙列缺损患者口腔种植后种植体松动度、牙周探诊深度(probing depth,PD)以及龈沟出血指数(sulcus bleeding index,SBI)的影响。方法:选择医院收治的慢性牙周炎牙列缺损并进行微创环切技术种植修复的55例患者(种植体86枚)为研究组;选择牙周健康的牙列缺损并进行微创环切技术种植修复的55例患者(种植体82枚)为对照组。随访12个月,比较两组患者口腔种植后种植体松动度、PD以及SBI指数情况,并评价两组患者生活质量。结果:术后12个月,研究组种植体松动度、留存率以及脱落率与对照组比较,差异均不存在统计学意义(P>0.05)。术后6个月,研究组PD、SBI与对照组比较,差异均不存在统计学意义(P>0.05);术后12个月,研究组PD、SBI均明显大于对照组,并大于术后6个月(P<0.05)。术后12个月,研究组口腔健康影响程度量表(oral health impact profile-14,OHIP-14)评分明显高于对照组(P<0.05)。结论:微创环切技术对牙列缺损患者具有良好的短期疗效,其对慢性牙周炎牙列缺损患者口腔种植后种植体松动度、PD以及SBI指数的近期影响与牙周健康患者无明显差异。 Objective:To explore the effects of minimally invasive circumcision on implant looseness,periodontal probing depth(PD)and sulcus bleeding index(SBI)in patients with dentition defects after oral implantation.Methods:Fifty-five patients(86 implants)with chronic periodontitis and dentition defects who were treated by the hospital and implanted with minimally invasive circumcision were selected and included in study group,and fifty-five patients(82 implants)with periodontal healthy dentition defects who were subjected to minimally invasive circumcision for implant restoration were selected and included in control group.After 12 months of follow-up,the implant looseness,PD and SBI index were compared between the two groups after oral implantation,and the quality of life of the two groups was evaluated.Results:At 12 months after surgery,there were no significant differences in implant looseness,retention rate and shedding rate between study group and control group(P>0.05).At 6 months after surgery,there were no statistically significant differences in the PD and SBI between study group and control group(P>0.05).At 12 months after surgery,the PD and SBI in study group were significantly greater than those in control group,and were significantly greater than those at 6 months after surgery(P<0.05).At 12 months after surgery,the oral health impact profile-14(OHIP-14)score in study group was significantly higher than that in control group(P<0.05).Conclusion:Minimally invasive circumcision has a good short-term efficacy on patients with dentition defects.Its short-term effects on implant looseness,PD and SBI index in patients with chronic periodontitis and dentition defects after oral implantation are not significantly different from those of periodontal healthy patients.
