上颌窦底黏膜增厚对上颌窦内提升术同期种植效果的影响

The impact of thickened sinus membrane on internal maxillary sinus floor lift and simultaneous implant

目的探讨上颌窦黏膜增厚时行上颌窦内提升术同期种植的临床效果及对种植体成功率的影响。方法选取15例上颌后牙缺失骨量不足,伴有上颌窦黏膜增厚>2 mm且术前均无明显上颌窦炎症的患者,行上颌窦内提升同期植入种植体18颗,术前、术后及术后6个月通过CBCT记录比较上颌窦黏膜厚度变化,术后6个月Ⅱ期修复,修复后追踪观察24个月,观察骨结合及行使功能情况,评估种植体成功率。结果15例患者共植入种植体18颗。CT检查术后4例上颌窦底提升后黏膜变薄,11例黏膜增厚,但均无上颌窦炎症状。术后半年,种植体均获得良好的骨结合,完成种植修复。修复后2年内定期回访,种植体周围骨结合及功能行使情况良好。结论上颌窦黏膜增厚对上颌窦内提升同期种植修复效果及成功率无影响。

Objective:To discuss the clinical effects and the success rate of implants after internal maxillary sinus floor lift with thickened sinus membrane and simultaneous implant.Methods:Fifteen patients with insufficient bone in the posterior zone of maxillary and with thickened sinus membrane more than 2mm were included in this study.All patients received sinus lift surgery and dental implants insertion.The change of the sinus membrane thickness was evaluated by CBCT pre-surgery,post-surgery and 6 months after surgery.crowns were made for all patients in the 6 months after surgery.There were Follow-up 24 months after clinical function recovery.The condition of the bone regeneration and the masticatory function were observed and the success rate of implants was evaluated.Results:The thickness of maxillary sinus membrane was decreased in 4 cases after sinus lift and increased in 11 cases.All patients had no maxillary sinusitis and infection.All the dental implants succeeded.Conclusion:Thickened maxillary sinus membrane has no influence on the clinical curative effect or the success rate of implant after internal maxillary sinus floor lift with and simultaneous implant.
