15389例Ⅰ类切口手术及血管介入操作围手术期预防性应用抗菌药物调查与分析

Investigation and Analysis of Perioperative Prophylactic Application of Antibiotics in 15389 Cases of TypeⅠIncision and Vascular Intervention Surgery

目的:了解宁夏回族自治区人民医院(以下简称“我院”)Ⅰ类切口手术及血管介入操作围手术期预防性应用抗菌药物的情况,为抗菌药物的临床应用与管理提供参考。方法:采用回顾性调查研究方法,调取2019年我院所有Ⅰ类切口手术及血管介入操作患者病历,对患者性别、年龄、住院时间、抗菌药物使用、品种选择、给药时间、用法与用量、疗程、合理用药点评以及手消毒考核等进行回顾性统计分析,并评价用药合理性及手消毒考核合格率。结果:共纳入15389例Ⅰ类切口手术及血管介入操作患者的病历,其中3823例预防性使用抗菌药物,抗菌药物预防性使用率为24.84%。抽样点评1120份病历,其中999份病历合理,合理率为89.20%。累计抽查837名外科医师手消毒考核,合格医师811名,手消毒考核合格率为96.89%。结论:我院Ⅰ类切口手术及血管介入操作围手术期预防用药基本合理,但仍存在一些问题,有待进一步规范化管理,提高合理用药水平。

OBJECTIVE:To investigate the prophylactic application of antibiotics in typeⅠincision and vascular intervention surgery in the People’s Hospital of Ningxia Hui Autonomous Region(hereinafter referred to as“our hospital”),so as to provide references for clinical drug application and management of antibiotics.METHODS:Retrospectively investigation and research was adopted,medical history of all typeⅠincision and vascular intervention surgery in our hospital in 2019 were retrieved and retrospectively analyzed in terms of patients’gender,age,hospital stays,application of antibiotics,selection of category,administration time,usage and dosage,course of treatment,review on rational drug application and hand disinfection test,so as to evaluate the rationality of drug application and qualification rate of hand disinfection.RESULTS:Totally 15389 cases of medical history of typeⅠincision and vascular intervention surgery were involved,of which 3823 cases had prophylactically applied antibiotics,with the application rate of antibiotics was 24.84%.1120 pieces of medical history were sampled for reviewing,of which 999 pieces were rational,with the rational rate was 89.20%.A total of 837 surgeons were randomly selected for hand disinfection test,811 were qualified,with the qualification rate of hand disinfection was 96.89%.CONCLUSIONS:The prophylactic application of antibiotics in typeⅠincision and vascular intervention surgery in our hospital is basically rational,while But are still some problems need to be further standardized management to improve the rational drug application.
