面部埋线提升术联合填充注射技术在下庭的肌肉调节 被引量:2

摘要 目的以肌肉动力学指导面部埋线提升术联合填充注射技术在下庭的肌肉调节。方法以面部下庭的肌肉群为基础分析肌肉动力学;以可注射透明质酸和可吸收缝线为主要材料为就诊者实施面部肌肉调节。结果肌肉动力学分为长度紧张关联,肌肉滑轮和杠杆系统,肌肉拮抗作用。透明质酸于皮下注射可增加肌肉长度;提肌下深层脂肪注射可改变杠杆滑轮系统支点;降肌上或浅层脂肪注射可增加肌肉阻力,实现调节肌肉作用。可吸收缝线可固定肌肉长度;聚拢深层组织,固定肌肉拉力或末端组织,实现调节肌肉作用。本案患者经SMAS层高位线性提拉、在颧大肌低位,蜗轴外上方的浅脂肪的深层施打透明质酸,获得满意效果。结论面部埋线提升术和可填充注射技术均具有调节面部下庭肌肉作用。 Objective Based on myodynamics theory to perform lower face lifting by using threads and filler injection.Method Clearly analyse the myodynamics based on lower facial mimetic muscles and use hyaluronic acids and absorbable threads to perform myomodulation on lower face.Results The myodynamics theory of facial mimetic muscles includes:1.length-tension relationship,2.muscle pulley and lever systems,3.functional muscle groups.Proper filler injection in the subcutis can increase muscle length;muscle pulley and lever systems would be changed when filler injected below the levator muscles;muscle obstacle force would be increased when filler injected on depressor muscles or the overlying superficial fat pad.Absorbable bi-directional cogged threads lifting techniques can fix the muscle length,concentrate the surrounding soft tissue and anchor the terminal fibrous tissue of the muscles(target point).We can achieve myomodulation by the methods mentioned above.We performed high SMAS thread lifting techniques and hyaluronic acid injection over the lower lateral edge of zygomaticus major muscle(the superficial fat pad on the upper-lateral area of modiolus)to achieve myomodulation and satisfying results.
