V-Y内眦赘皮矫正术与Z成形术分别联合park法重睑成形术治疗单睑伴内眦赘皮患者的效果比较

Comparison of the effects of V-Y epicanthus correction and Z plasty combined with park method double eyelid plasty on patients with single eyelid and epicanthus

目的比较V-Y内眦赘皮矫正术与Z成形术分别联合park法重睑成形术治疗单睑伴内眦赘皮患者的临床效果。方法选取2018年1月~2019年10月我院单睑伴内眦赘皮患者97例,按随机数字表法分V-Y矫正组(n=49)、Z成形组(n=48)。均行park法重睑成形术治疗,在此基础上,V-Y矫正组实施同期V-Y内眦赘皮矫正术,Z成形组实施Z成形术。比较两组美学总有效率、术前、术后3个月眼裂变化情况(眼裂长度、倾斜度、眼裂角)、术后1个月、3个月、6个月瘢痕严重程度(VSS评分)、术后并发症发生率。结果两组总有效率比较差异无统计学意义(P>0.05);术后3个月两组眼裂长度明显变长,V-Y矫正组长于Z成形组,倾斜度、眼裂角减小,V-Y矫正组小于Z成形组(P<0.05);术后1个月两组VSS评分比较差异无统计学意义(P>0.05),术后3个月、6个月V-Y矫正组VSS评分均低于Z成形组(P<0.05);V-Y矫正组术后并发症发生率低于Z成形组(P<0.05)。结论V-Y内眦赘皮矫正术与Z成形术分别联合park法重睑成形术治疗单睑伴内眦赘皮均可获得良好美学疗效,但V-Y内眦赘皮矫正术在眼裂改善、瘢痕消退等方面更具优势,且可降低术后并发症风险。

Objective To compare the clinical effects of V-Y epicanthus correction and Z plasty combined with park method double blepharoplasty in the treatment of patients with single eyelid and epicanthus.Methods From January 2018 to October 2019,97 patients with single eyelid and epicanthus were selected and divided into V-Y correction group(n=49)and Z plasty group(n=48)according to the random number table method.All of them were treated with park method for double eyelid plasty.On this basis,the V-Y correction group implemented the same period of V-Y epicanthus correction,and the Z plastic group performed Z plastic surgery.Comparing the total aesthetic efficiency,the changes in eye cracks(eye crack length,inclination,eye crack angle)before and 3 months after surgery,the severity of scars(VSS score)at 1 month,3 months,and 6 months after surgery,and the incidence of postoperative complications of the two groups.Results There was no significant difference in the total effective rate between the two groups(P>0.05);three months after surgery,the length of the eye cracks in both groups was significantly longer.The V-Y correction group was longer than the Z plasty group,and the inclination and eye crack angle were reduced.The V-Y correction group was smaller than the Z plasty group(P<0.05);there was no significant difference in VSS score between the two groups at 1 month after operation(P>0.05),at 3 and 6 months after surgery,the V-Y correction group had lower VSS scores than the Z plasty group(P<0.05);the incidence of postoperative complications in the V-Y correction group was lower than that in the Z plasty group(P<0.05).Conclusion Both V-Y epicanthus correction and Z plasty combined with park method double eyelid plasty for single eyelid with epicanthus can obtain good aesthetic effects.However,V-Y epicanthus correction has advantages in improving eye cracks and scars,and can reduce the risk of postoperative complications.
