小腿远端蒂穿支皮瓣修复足踝部软组织缺损对皮瓣切取面积、修复效果的影响

Influences of distally-based sural perforator flap repairing soft tissue defects of ankle on area of incised flaps and repair effect

目的观察小腿远端蒂穿支皮瓣修复足踝部软组织缺损对皮瓣切取面积、修复效果。方法选取2017年5月~2019年5月收治的89例的足踝部软组织缺损患者为研究对象,足踝组织缺损部位包括踝前联合跗内、外侧区31例、背跗外侧区23例、踝前区20例、跗外侧区联合跖背区15例。结果术后6例(6.74%)患者出现张力性水泡,皮瓣经局部减压后完全存活;全部患者术后均未发现渗出、溃疡,且皮瓣恢复良好;全部患者术后1年踝前联合跗内、外侧区、背跗外侧区、踝前区、跗外侧区联合跖背区的腓动脉穿支皮瓣均完全存活,存活率达100%。结论小腿远端蒂穿支皮瓣切取面积可完全覆盖足踝部软组织缺损范围,且修复效果良好。

Objective To observe the influences of distally-based sural perforator flap repairing soft tissue defects of ankle on area of incised flaps and repair effect.Methods Eighty-nine patients with soft tissue defects of ankle who were admitted from May 2017 to May 2019 were enrolled as the research objects.There were 31 cases with soft tissue defects at anterior ankle combined with medial and lateral tarsea areas,23 cases at lateral dorsal tarsea area,20 cases at anterior ankle area,and 15 cases at lateral tarsea area combined with dorsal metatarsal area.Results After surgery,there were 6 patients(6.74%)with tension blisters,whose flaps were survived after partial decompression.There was no exudation or ulcer any patient,and recovery of the flaps was good.At 1 year after surgery,sural perforator flaps at anterior ankle combined with medial and lateral tarsea areas,lateral dorsal tarsea area,anterior ankle area,and lateral tarsea area combined with dorsal metatarsal area were all survived completely,with survival rate of 100%.Conclusion The incised area of distally-based sural perforator flaps can completely cover the soft tissue defects site of ankle,with good repair effect.
