心理干预对维持性血液透析患者不良事件发生情况及生活质量的影响

Effect of psychological intervention on occurrence of adverse events and quality of life in maintenance hemodialysis patients

目的探讨心理干预对维持性血液透析患者不良事件及生活质量的影响。方法选取2018年11月至2020年2月中国人民解放军总医院第七医学中心血液净化中心收治的110例常规维持性血液透析患者,按照随机数字表法分为观察组和对照组,各55例。对照组行常规方法干预,观察组在对照组基础上给予心理干预。比较2组患者干预前后汉密尔顿抑郁量表和汉密尔顿焦虑量表评分、不良事件发生情况以及生活质量量表评分。结果干预后,观察组患者的焦虑评分和抑郁评分均明显低于对照组[(4.4±1.7)分比(6.5±2.2)分;(5.0±1.0)分比(7.2±2.6)分],差异均有统计学意义(均P<0.05)。观察组不良事件发生率明显低于对照组[9.1%(5/55)比32.7%(18/55)],差异有统计学意义(P<0.001)。观察组心理干预后生活质量量表各项目评分均明显高于对照组,差异均有统计学意义(均P<0.05)。结论心理干预可有效排解维持性血液透析患者的负面情绪,降低其不良事件发生率,提高其生活质量。

Objective To explore the effect of psychological intervention on the adverse events and quality of life in patients with maintenance hemodialysis.Methods From November 2018 to February 2020,110 patients with routine maintenance hemodialysis admitted to the Seventh Medical Center of Chinese PLA General Hospital were randomly divided into observation group and control group,with 55 cases in each group.The control group was treated with routine intervention,and the observation group was given psychological intervention on the basis of the control group.The scores of Hamilton depression scale and Hamilton anxiety scale,adverse events and quality of life scale were compared between the two groups before and after intervention.Results After intervention,the scores of anxiety and depression in observation group were significantly lower than those in control group[(4.4±1.7)vs(6.5±2.2);(5.0±1.0)vs(7.2±2.6),both P<0.05].The incidence of adverse events in observation group was significantly lower than that in control group[9.1%(5/55)vs 32.7%(18/55),P<0.001].The scores of each item of quality of life scale in observation group were significantly higher than those in control group(all P<0.05).Conclusion After receiving psychological care,the negative emotions of the patients with maintenance hemodialysis are eliminated to a large extent,the incidence of adverse eventsis reduced,and the quality of life is improved.
