三皮汤治疗儿童咳嗽变异性哮喘的临床疗效及对肺功能、生活质量的影响

Clinical efficacy of the Sanpi decoction on children with cough variant asthma and lung function with quality of life

目的:探讨三皮汤治疗儿童咳嗽变异性哮喘(Cough Variant Asthma,CVA)的临床疗效。方法:选取我院收治的82例符合入组标准的儿童咳嗽变异性哮喘作为研究对象,按随机数字表分为对照组和观察组,每组各41例。对照组给予常规西药丙酸氟替卡松吸入气雾剂和孟鲁司特钠治疗,观察组在对照组的基础再口服三皮汤,两组均以14 d为一个疗程,均治疗2个疗程。治疗前及治疗后采用肺功能仪检测肺功能指标[一秒用力呼气容积(Forced Expiratory Volume in 1 second,FEV1)、第一秒用力呼气量占用力肺活量的比值(FEV1/FVC)、最大呼气流量占正常预计值的百分比(PEF%)],采用儿童少年生活质量问卷进行生活质量评价,评价两组临床疗效,记录不良反应发生情况。结果:对照组和观察组治疗2个疗程后FEV1、FEV1/FVC、PEF%均较治疗前明显升高,P<0.05,观察组治疗2个疗程后FEV1、FEV1/FVC、PEF%明显高于对照组,P<0.05。对照组和观察组治疗后症状评分均较治疗前明显降低,P<0.05,观察组治疗后症状评分明显低于对照组,P<0.05。观察组治疗2个疗程的临床总有效率高于对照组,差异具有统计学意义,χ^2=12.007,P<0.05。对照组患儿咳嗽复发时间低于观察组,差异具有统计学意义,t=3.821,P<0.05。对照组和观察组治疗2个疗程后自我满意度、躯体感受、运动能力、学习能力与态度评分均较治疗前明显升高,P<0.05,观察组治疗2个疗程后自我满意度、躯体感受、运动能力、学习能力与态度评分明显高于对照组,P<0.05。两组患儿治疗过程未见明显不良反应。结论:三皮汤治疗儿童咳嗽变异性哮喘具有显著效果,能有效改善咳嗽症状及肺功能,延长咳嗽复发间隔时间,提高患儿生活质量,且安全性良好。

Objective:To investigate the clinical efficacy of the Sanpi decoction(三皮汤)on cough variant asthma.Methods:82 cases were divided into the control group and observation group,with 41 cases in each group.The control group was treated with fluticasone propionate inhalation aerosol and montelukast sodium,and the observation group was treated with the Sanpi decoction more.Results:FEV1,FEV1/FVC,PEF%after 2 courses of treatment in groups were significantly higher than before treatment,P<0.05,and FEV1,FEV1/FVC,PEF%in the observation group were significantly higher than the control group,P<0.05.The symptom scores in groups after treatment were significantly lower than those before treatment,P<0.05,and the symptom scores in the observation group after treatment were significantly lower than those in the control group,P<0.05.The total clinical effective rate in the observation group was higher than the control group,χ^2=12.007,P<0.05.The cough recurrence time of children in the control group was lower than that in the observation group,t=3.821,P<0.05.The scores of self-satisfaction,somatic feeling,exercise ability,learning ability and attitude in groups were significantly higher than those before treatment,P<0.05.The scores of self-satisfaction,somatic feelings,sports ability,learning ability and attitude were significantly higher than those in the control group,P<0.05.There was no obvious adverse reactions in the treatment process of the two groups of children.Conclusion:The Sanpi decoction has a significant effect on cough variant asthma in children.It can effectively improve cough symptoms and lung function,prolong the interval between cough recurrence,improve the quality of life of children,and is safe.
