尤氏卵巢早衰方治疗肾虚血虚型卵巢早衰的临床疗效研究

The clinical study on treating premature ovarian failure of the Shenxu Xuexu type with the You’s premature ovarian failure prescription

目的:探究尤昭玲教授卵巢早衰经验方治疗肾虚血虚型卵巢早衰的临床疗效。方法:将我院2019年1-12月79例辨证为肾虚血虚型卵巢早衰患者做为研究对象,分为治疗组40例和对照组39例。对照组给予人工周期疗法(即补佳乐+黄体酮胶囊)治疗,治疗组给予尤氏经验方治疗。两组均治疗3个周期。观察两组治疗前及治疗3个月后月经改善情况,中医证候改善情况,观察两组治疗前及停药后3个月生殖内分泌激素变化情况。观察两组治疗前及治疗后3个月阴道B超下子宫及卵巢超声变化情况。结果:治疗组月经症状改善的有效率优于对照组,差异有统计学意义(P<0.01);治疗组中医证候改善情况的有效率高于对照组,两组比较,差异有统计学意义(P<0.01);停药3个月后,两组雌二醇(Estradiol,E2)水平均明显升高(P<0.05),血清卵泡刺激素(Follicle Stimulating Hormone,FSH)、黄体生成素(Luteinizing Hormone,LH)水平均明显降低(P均<0.05),且治疗组的各项激素改善情况均优于对照组(P<0.05);治疗3个月后,两组患者卵巢体积均增大,子宫内膜厚度均增厚,但治疗组子宫血流状况明显高于对照组,阻力指数显著降低,两组之间的差异均有统计学意义(P<0.01)。结论:尤氏经验方治疗卵巢早衰能有效改善月经情况,及其相关中医症状,改善生殖内分泌激素水平及子宫内膜厚度,能提高临床疗效,值得临床推广运用。

Objective:To observe the clinical effects of the You’s premature ovarian failure prescription on premature ovarian failure of the Shenxu Xuexu type(肾虚血虚型).Methods:79 cases in our hospital were studied and divided into the treatment group(40 cases)and the control group(39 cases).The control group was given artificial cycle therapy,namely Bujiale(补佳乐)and progesterone capsule treatment,while the treatment group was given You’s empirical prescription.Both groups were treated for 3 cycles.Results:After treatment,the efficiency of menstrual improvement in the treatment group was higher than the control group(P<0.01).Improvement of TCM syndromes in the treatment group was better than the control group(P<0.01).After 3 months of withdrawal,the levels of E2 were significantly increased in both groups(P<0.05),and the levels of FSH and LH were both significantly decreased in both groups(P<0.05),and the improvement of various hormones in the treatment group was better than the control group(P<0.05).After 3 months of treatment,the ovarian volume in both groups was increased,endometrial thickness was thickened,but the uterine blood flow in the treatment group was significantly better than the control group,resistance index was significantly decreased(P<0.01).Conclusion:The empirical prescription of Professor You ZhaoLing for treating premature ovarian failure can effectively improve menstrual status and related TCM symptoms,improve the level of reproductive endocrine hormone and endometrium thickness,and improve clinical efficacy,and is worthy of clinical promotion and application.
