外固定架治疗青藏高原地区四肢骨创伤的临床特点及治疗效果分析

Clinical Characteristics and Therapeutic Effect of External Fixator in the Treatment of Limb Bone Trauma in Qinghai Tibet Plateau

目的针对四肢骨创伤患者,研究分析外固定架治疗的临床特点及治疗效果。方法以2018年1月至2019年12月我院骨科收治的总计78例四肢骨创伤患者为对象,随机抽取以每组39例作为对照组和观察组,实施传统内固定治疗针对对照组,外固定架治疗针对观察组,将两组的临床指标、临床疗效以及并发症情况做比较分析,评价外固定架治疗的临床效果。结果两组的临床指标(手术用时、术中出血量、愈合时间以及住院日)中观察组均明显优于对照组,差距有意义(P<0.05)。两组的临床疗效中观察组(总有效率97.44%)明显优于对照组(总有效率82.05%),差距有意义(P<0.05)。观察组治疗后出现术后感染患者1例,关节僵硬患者1例,不愈合患者1例;对照组治疗后出现术后感染患者3例,关节僵硬患者2例,皮肤坏死患者2例,不愈合患者4例。两组的并发症情况中观察组(总发生率7.69%)明显低于对照组(总发生率28.21%),差距有意义(P<0.05)。结论外固定架治疗能提高四肢骨创伤患者的治疗效果,提高了多发伤危重症合并四肢骨创伤患者的救治率,符合损伤控制理论,加快身体机能的恢复,提高了开放性粉碎性四肢骨创伤的骨折愈合率,同时减少了并发症,值得推广。

Objective According to the patients with limb bone trauma,the clinical characteristics and therapeutic effect of external fixator were analyzed.Methods From January 2018 to December 2019,78 patients with limb bone trauma included in orthopedics department of our hospital were selected randomly as the observation group and the control group,39 patients in each group were treated with traditional internal fixation for the control group and external fixation for the observation group.The clinical indicators,clinical efficacy and complications of the two groups were compared and analyzed to evaluate the clinical effect of external fixation.Results The clinical indexes of the two groups(operation time,bleeding volume,healing time and hospital stay)were significantly better than those of the control group(P<0.05).In the clinical efficacy of the two groups,the observation group(total effective rate 97.44%)was significantly better than the control group(total effective rate 82.05%),the difference was significant(P<0.05).In the observation group,there were 1 case of postoperative infection,1 case of joint stiffness and 1 case of nonunion.In the control group,there were 3 cases of postoperative infection,2 cases of joint stiffness,2 cases of skin necrosis and 4 cases of nonunion.In the complications of the two groups,the incidence of the observation group(7.69%)was significantly lower than that of the control group(28.21%),and the difference was significant(P<0.05).Conclusion External fixator treatment can improve the treatment effect of limb bone trauma patients,and improve the treatment rate of multiple trauma patients with limb bone trauma,which is in line with the damage control theory.Accelerate the recovery of body function,improve the fracture healing rate of open comminuted limb bone trauma,and reduce the complications.It is worth popularizing.
