对行腹腔镜子宫肌瘤剔除术治疗患者采用纳布啡联合舒芬太尼进行术后镇痛的效果

Effect of Postoperative Analgesia of Patients Treated by Laparoscopic Myomectomy with Nabulphine combined with Sufentanil

目的探究子宫肌瘤患者行腹腔镜子宫肌瘤剔除术治疗后联合应用纳布啡与舒芬太尼进行术后镇痛的临床效果。方法选取我院2017年1月至2020年1月期间收治的42例子宫肌瘤患者作为研究对象,所有患者均行腹腔镜子宫肌瘤剔除术治疗。术后依据镇痛方式的不同进行分组,将单一应用舒芬太尼的患者设为单一组(21例),而联合应用舒芬太尼与纳布啡的患者设为联合组(21例)。评估两组术后不同时间(2h、6h、12h与24h)疼痛程度与镇痛效果,并记录两组不良反应(恶心呕吐、头晕)发生率。结果术后2h,两组视觉模拟疼痛(VAS)评分基本一致(P>0.05);术后6h、12h与24h,两组VAS评分均呈下降趋势,对比发现联合组降幅更大(P<0.05);在术后不同时间比较上,两组Ramsay评分无显著差异(P>0.05)。在恶心呕吐、头晕发生率比较上,两组不存在显著差异(P>0.05)。结论在腹腔镜子宫肌瘤剔除术治疗后,联合应用纳布啡与舒芬太尼做术后镇痛处理,可避免对镇静效果造成影响,且能够显著改善患者疼痛感,不良反应发生率低。

Objective To investigate the clinical effect of postoperative analgesia in patients with uterine fibroids treated with laparoscopic myomectomy combined with nabulphine and sufentanil.Methods 42 patients with uterine fibroids admitted to our hospital from January 2017 to January 2020 were selected as the research objects.All patients were treated with laparoscopic myomectomy.According to the different analgesic methods,the patients treated with sufentanil alone were divided into a single group(21 cases),while the patients treated with sufentanil combined with nabulphine were divided into a combined group(21 cases).To evaluate the pain degree and analgesic effect at different time(2h,6h,12h and 24h).Results At 2h after surgery,visual analogue pain(VAS)scores of the two groups were basically consistent(P>0.05).At 6h,12h and 24h after surgery,VAS scores in both groups showed a downward trend,and the comparison showed that the decline was greater in the combined group(P<0.05).There was no significant difference in Ramsay scores between the two groups at different postoperative times(P>0.05).There was no significant difference in the incidence of nausea,vomiting and dizziness between the two groups(P>0.05).Conclusion After laparoscopic hysteromyomectomy,combined use of nabphine and sufentanil for postoperative analgesia treatment can avoid the influence on sedative effect,and can significantly improve the patient’s pain,with a low incidence of adverse reactions.
