采用心率震荡(HRT)参数评估阻塞性睡眠呼吸暂停低通气综合征(OSAHS)对高血压患者心脏自主神经功能的影响

Using Heart Rate Turbulence(HRT)Parameters to Evaluate the Effect of Obstructive Sleep Apnea Hypopnea Syndrome(OSAHS)on Cardiac Autonomic Nerve Function in Patients with Hypertension

目的利用HRT参数了解OSAHS对高血压心脏自主神经功能的影响。方法选取2017年12月—2019年11月期间在该院治疗的怀疑合并OSAHS的102例患者,进行PSG监测、DCG监测,将其分为EH组68例及OSA-RH组34例,分析两组患者的一般临床资料,并进一步分析HRT参数与AHI的相关性。结果OSA-RH组患者心率、AHI、TO等指标均明显高于EH组(P<0.05);OSA-RH组患者0级比例明显低于EH组(23.53%vs 70.59%),1级(50.00%vs 19.12%)、2级(26.47%vs 10.29%)比例明显高于对照组(Z=8.480,P<0.05);相关性分析结果显示,AHI与TO、HRT参数危险分层存在显著正相关(r=0.882、0.779,P<0.05)。结论HRT参数与AHI具有相关性,HRT受损可能是诱发OSA-RH患者发生心血管不良事件的潜在危险因素。

Objective Use HRT parameters to understand the effect of OSAHS on hypertensive cardiac autonomic nerve function.Methods We selected 102 patients suspected of having OSAHS who were treated in the hospital from December 2017 to November 2019 for PSG monitoring and DCG monitoring.They were divided into 68 cases in the EH group and 34 cases in the OSA-RH group,and the two groups patients’general clinical data were analyzed,and further analyze the correlation between HRT parameters and AHI.Results The heart rate,AHI,TO and other indicators of the OSA-RH group were significantly higher than those of the EH group(P<0.05);the proportion of grade 0 patients in the OSA-RH group was significantly lower than that of the EH group(23.53%vs 70.59%).The proportions of grade 1(50.00%vs 19.12%)and grade 2(26.47%vs 10.29%)were significantly higher than those of the control group(Z=8.480,P<0.05);the results of the correlation analysis showed that AHI There was a significant positive correlation with AHI,TO and HRT parameters risk stratification(r=0.882,0.779,P<0.05).Conclusion HRT parameters are correlated with AHI.Impaired HRT may be a potential risk factor for inducing adverse cardiovascular events in patients with OSA-RH.
