螺纹克氏针顺行固定治疗指骨骨折的临床疗效研究

Study on the Clinical Effect of Antegrade Fixation with Threaded Kirschner Wire in the Treatment of Phalanx Fractures

目的研究螺纹克氏针顺行固定治疗指骨(趾骨)骨折的临床效果。方法选择2017年2月—2019年10月该院68例指骨骨折患者为对象,对其临床资料进行回顾性分析,患者均接受螺纹克氏针内固定治疗,分析治疗结果。结果该组68例患者骨折解剖复位率为95.59%,创面Ⅰ期愈合率为97.06%,骨折愈合平均时间(7.12±2.16)周;68例患者术后指关节主动活动度优良率为94.12%;68例患者术后关节功能恢复优良率为92.65%;68例患者术后对外观的满意度为100.00%。结论螺纹克氏针顺行固定治疗指骨骨折有常规克氏针的所有优点,另外还具备对骨折线施加压力,让骨折端紧密结合有利于骨性愈合,但螺纹克氏针使用技术要求及设备要求高,一般需在C臂机引导下一次穿针成功,不可反复穿针,因此未能在临床普及。

Objective To study the clinical effect of antegrade fixation with threaded Kirschner wire in the treatment of phalanx(phalangeal)fractures.Methods From February 2017 to October 2019,68 patients with phalanx fractures in the hospital were selected as subjects,and their clinical data were retrospectively analyzed.All patients received threaded Kirschner wire internal fixation and the treatment results were analyzed.Results The fracture anatomical reduction rate of 68 patients in this group was 95.59%,the first-stage wound healing rate was 97.06%,and the average fracture healing time was(7.12±2.16)weeks;the excellent and good rate of active knuckle range of motion in 68 patients was 94.12%.The excellent and good rate of postoperative joint function recovery in 68 patients was 92.65%;the satisfaction of 68 patients with appearance after surgery was 100.00%.Conclusion Threaded Kirschner wire anterograde fixation for the treatment of phalangeal fractures has all the advantages of conventional Kirschner wires.In addition,it also has all the advantages of conventional Kirschner wires to apply pressure to the fracture line,so that the fracture ends are tightly combined and are beneficial to bone healing.The equipment requirements are high,and the C-arm machine is generally required to guide the next needle penetration successfully.Repeated needle penetration is not allowed,so it has not been popularized in clinic.
