早期肠内营养支持在危重症病人中的应用

摘要 目的探讨早期肠内营养支持在危重症患者中应用效果。方法选取2017年3月至2019年4月我院ICU收治危重症患者80例实施研究,将患者用随机分组表分为基础组(基础治疗和基础营养支持,40例)和干预组(基础治疗上早期肠内营养支持,40例),比较两组患者治疗前后白蛋白水平、急性生理学和慢性健康状况评分(APACHEⅡ评分)及患者ICU住院时间和预后。结果治疗前,两组患者血清白蛋白、APACHEⅡ评分无明显差异(P>0.05);治疗7d,基础组患者血清白蛋白显著低于干预组,APACHEⅡ评分显著高于干预组(P<0.05);基础组ICU住院时间显著高于干预组(P<0.05)。基础组患者死亡率显著高于干预组(20.00%VS 50.0%,P<0.05)。结论早期肠内营养干预在危重症患者中应用,可协助改善患者预后,降低患者死亡率。 Objective to explore effect of early enteral nutrition support in critically ill patients.Methods choose 80 cases critically ill patients admitted to ICU of our hospital from March 2017 to April 2019 for study.Divide them into basic group(basic treatment and basic nutritional support,40 cases)and intervention group(basic early enteral nutrition support on treatment,40 cases).Compare albumin level,acute physiology and chronic health scores(APACHEⅡscores),ICU hospital stay and prognosis between two groups before and after treatment.Results before treatment,there was no significant difference in serum albumin and APACHEⅡscores between two groups(P>0.05);after 7 days of treatment,serum albumin in basic group was significantly lower than intervention group,and APACHEⅡscores were significantly higher than intervention group(P<0.05);length of stay in ICU of basic group was significantly higher than intervention group(P<0.05).Mortality rate in basic group was significantly higher than intervention group[20.00%VS 50.0%,P<0.05].Conclusion early enteral nutrition intervention can improve prognosis and reduce mortality of critically ill patients.
