老年慢性病患者门诊跌倒预防中实施健康指导护理分析

Health Guidance Nursing Analysis in Falls Prevention of Elderly Patients with Chronic Diseases

目的探究健康指导护理应用于老年慢性病患者门诊跌倒预防中的临床效果。方法将2019年5月至2020年4月我院门诊收治的老年慢性病病患98例,随机分为参照组和研究组各49例,参照组行常规护理,研究组在其基础上行健康指导护理。比较两组病患护理前后心理状态、日常生活能力改善情况,以及跌倒发生率、平均跌倒次数、防跌倒知识掌握情况以及对护理的满意度。结果两组病患护理前SDS、SAS和ADL评分比较差异无统计学意义(P>0.05),经护理后研究组病患SDS和SA评分明显低于参照组,ADL评分明显高于参照组;跌倒发生率8.16%明显低于参照组的26.53%,平均跌倒次数明显少于参照组,防跌倒知识掌握得分明显高于参照组;对护理的总满意率高达95.92%,明显高于对照组的75.51%,差异均有统计学意义(P<0.05)。结论健康指导护理应用于老年慢性病患者门诊跌倒预防中效果显著,可有效改善病患心理状态,提高其日常生活能力,从而有效预防跌倒情况发生,有利于增加病患防跌倒知识掌握情况,提高其对护理的满意度,具有较高的临床应用价值。

Objective to explore clinical effect of health guidance nursing for elderly patients with chronic disease during outpatient falls prevention.Methods choose 98 cases elderly patients with chronic diseases in our hospital from May 2019 to April 2020,divide them into reference group and study group randomly,with 49 cases in each group.Reference group was treated with routine nursing,and study group with health guidance nursing on the basis.Compare improvement of mental state,activities of daily living,incidence of falls,average number of falls,knowledge of fall prevention and nursing satisfaction between two groups before and after nursing.Results there was no significant difference in SDS,SAS and ADL scores between two groups before nursing(P>0.05).After nursing,SDS and SA scores of study group were significantly lower than reference group,and ADL scores were significantly higher than reference group;incidence rate of falls was 8.16%,which was significantly lower than 26.53%of reference group,average number of falls was significantly less than reference group,scores of knowledge mastery of falls prevention were significantly higher than reference group,satisfaction degree was 95.92%,which was significantly higher than 75.51%of control group(P<0.05).Conclusion health guidance nursing can achieve significant effect for elderly patients with chronic disease during outpatient falls prevention,improve psychological state,ability of daily life effectively,prevent falls incidence effectively,increase their knowledge of falls prevention and improve nursing satisfaction deree,which is of high clinical application value.
