新冠肺炎定点医院感染控制策略

Study on the management strategy of nosocomial infection in the epidemic of COVID-19

目的:制定便于实施的管理方案,避免疫情时期院内交叉感染的发生。方法:通过持续改进制度流程、隔离病区空间布局、医务人员个人防护、健康管理以及消毒隔离技术等,最终形成"平疫结合、分层管控"的管理策略。结果:2020年1月20日-9月9日,上海市公共卫生临床中心共收治确诊新型冠状病毒肺炎患者890例,治愈出院842例,参与应急救治的一线医务人员共700人(1 322人次),其中外院支援医务人员共347人(470人次)。在此期间普通门诊量共计201 961人次,住院人数为12 474人次,复工复产过程中该院普通患者平均在院人数为800~900人/d,日门诊量1 300余人次。医务人员"零"感染,无院内交叉感染事件发生。结论:利用"平疫结合,分层管控"的管理方案,在疫情中以及"后疫情时代",可以有效避免医院感染的发生,保障医疗安全。

Objective:To formulate a management strategy that is easy to implement and avoid nosocomial infection in the epidemic of COVID-19.Methods:The strategy of "combination normal condition with emergency and hierarchical management control" was finally formed by continuous improvement of system process,spatial layout of isolated disease area,personal protection of medical personnel,health management and disinfection and isolation technology.Results:Eight hundred and ninety cases of COVID-19 were diagnosed and admitted from January 20 to September 9,2020,in which 842 cases were cured.A total of 700(1 322 person times) medical staff participated in the emergency treatment work,in which 347 staff(470 person times) were supported by other hospitals.During this period,the total number of general outpatient services was 201 961,and the number of inpatients was 12 474.During the process of resuming work and production,the average number of general patients in the hospital was 700~800 per day,and the daily number of outpatient services was more than 1 300.None of medical staff was infected and no hospital cross infection occurred.Conclusion:The management plan of"combination normal condition with emergency and hierarchical management control" can effectively avoid nosocomial infection and ensure medical safety in the response and late stage of the epidemic.
