止血散瘀明目汤联合羟苯磺酸钙治疗脉络瘀滞型糖尿病视网膜病变疗效及安全性研究

Study on Clinical Efficacy and Safety of Zhixue Sanyu Mingmu Decoction Combined with Calcium Hydroxybenzoate in Treatment of Diabetic Retinopathy with Blood Stasis in Collateral

目的探究止血散瘀明目汤联合羟苯磺酸钙治疗脉络瘀滞型糖尿病视网膜病变临床疗效及安全性。方法研究共计纳入108例脉络瘀滞型糖尿病视网膜病变患者,均由医院门诊收治,采取随机数字表法将其分为两组,各54例,对照组常规西药治疗,观察组在对照组患者治疗基础上联合止血散瘀明目汤和羟苯磺酸钙治疗,比较两组患者治疗效果、治疗前后中医症状积分、患者黄斑厚度、出血斑及视野灰度值、视力变化情况及视网膜电生理结果、血清C反应蛋白(hs-CRP)、血管内皮生长因子(VEGF)、类胰岛素一号增长因子(IGF-1)水平变化、血液流变学指标变化、治疗不良反应发生率。结果观察组患者治疗有效率96.30%(52/54)高于对照组83.33%(45/54)(P<0.05);治疗前,两组患者中医症状积分、黄斑厚度、出血斑及视野灰度值、视力、视网膜电生理结果、hs-CRP、VEGF、IGF-1、血液流变学指标均相当(P>0.05),治疗后各组患者中医症状积分、黄斑厚度、出血斑及视野灰度值、视力、视网膜电生理结果、hs-CRP、VEGF、IGF-1、血液流变学指标均改善,观察组改善优于对照组(P<0.05);观察组患者不良反应率(3.70%)与对照组(1.85%)均较低(P>0.05)。结论止血散瘀明目汤联合羟苯磺酸钙治疗脉络瘀滞型糖尿病视网膜病变临床疗效佳,不良反应率低,安全可靠,值得推广以及应用。

Objective To explore the clinical efficacy and safety of Zhixue Sanyu Mingmu Decoction combined with calcium hydroxybenzoate in the treatment of diabetic retinopathy with blood stasis in collateral.Methods A total of 108 patients with diabetic retinopathy with blood stasis in collateral were included in the study.All the patients were admitted to the outpatient department of our hospital.The random number table method was adopted to divide them into two groups,54 cases in each.The control group received conventional western medicine treatment.The observation was treated with Zhixue Sanyu Mingmu Decoction combined with calcium hydroxybenzoate.We compared two groups’traditional Chinese medicine symptom integral,macular thickness,plaque and horizons grey value,the results of the vision and the changes of retinal electrophysiology,serum levels of C-reactive protein(hs-CRP)and vascular endothelial growth factor(VEGF)and insulin-like growth factor 1(IGF-1)and blood rheology index changes before and after treatment.And the incidence of adverse reactions was compared.Results The effective rate of treatment in the observation group(96.30%)was higher than that in the control group(83.33%,45/54)(P<0.05).Beforetreatment,two groups’TCM symptom integral,macular thickness,plaque and vision grayscale values,vision,retinal electrophysiology results,levels of hs-CRP,VEGF,IGF-1 and hemorheology indexes were not significant(P>0.05).The improvement of the observation group was better than that of control group after treatment(P<0.05).The rate of adverse reactions in the observation group(3.70%)was lower than that of the control group(1.85%)without significance(P>0.05).Conclusion Zhixue Sanyu Mingmu Decoction combined with calcium hydroxybenzoate has safe and reliable effect in the treatment of diabetic retinopathy with blood stasis in collateral as well as low adverse reaction rate.It is worthy of promotion and application.
