后路椎弓根钉棒内固定手术治疗脊柱胸腰段骨折的临床疗效评价

Clinical Evaluation of Posterior Pedicle Screw Rod Internal Fixation for Thoracolumbar Fracture

目的对于我院收治的脊柱胸腰段骨折患者,探究后路手术治疗的整体临床效果。方法将2017年1月至2019年10月到我院接受治疗的胸腰段骨折患者135例纳入本研究,根据实施治疗方式的不同分为两组,67例保守组(保守治疗)与68例手术组(后路手术),分别对比两组患者治疗前后椎体各项指标改变情况、疼痛程度以及整体临床效果。结果两组患者椎体各项指标改变和VAS评分有可比性(P>0.05);与手术组患者相对比,保守组患者Cobb角显著偏高,而前后缘椎体高度比显著偏低,VAS评分显著偏高(P<0.05);与手术组患者相对比,保守组患者总临床疗效显著偏低,两组间数据差异具有统计学意义(P<0.05)。结论与保守治疗脊柱胸腰段骨折患者整体疗效相对比,后路椎弓根钉棒内固定手术治疗效果更为显著。经手术治疗后脊柱胸腰段骨折患者疼痛程度得以缓解,伤椎高度显著恢复,降低后凸角度丢失。

Objective To explore the overall clinical effect of posterior surgical treatment for spinal thoracolumbar fracture patients admitted to our hospital.Methods From January 2017 to October 2019,135 patients with thoracolumbar fracture who had been treated in our hospital were included in this study.According to the different ways of treatment,they were divided into two groups:67 patients in the conservative group(conservative treatment)and 68 patients in the operation group(posterior operation).The changes of vertebral body indexes,the degree of pain and the overall clinical effect of the two groups were compared before and after treatment.Results The changes of vertebral body indexes and VAS scores in the two groups were comparable(P>0.05).Compared with the patients in the operation group,the Cobb angle of the patients in the conservative group was significantly higher,while the height ratio of the anterior and posterior vertebral bodies was significantly lower,and the VAS score was significantly higher.The data difference between the two groups is statistically significant(P<0.05).Compared with the patients in the operation group,the total clinical efficacy in the conservative group is significantly lower.The data difference between the two groups is statistically significant(P<0.05).Conclusion Compared with conservative treatment of thoracolumbar fracture,posterior pedicle screw rod internal fixation is more effective.After operation,the degree of pain of the patients with thoracolumbar fracture was relieved,the height of the injured vertebra was significantly restored,and the loss of kyphosis angle was reduced.
