摘要
目的探讨水通道蛋白-9(AQP-9)、缺氧诱导因子-1α(HIF-1α)在妊娠高血压综合征(妊高征)产妇胎盘组织中的表达及与产妇临床特征的相关性。方法选取2017年1月至2019年1月于川北医学院附属医院产科住院的90例妊高征产妇作为研究对象。根据症状类型分为妊娠高血压组(n=30)、轻度子痫前期组(n=30)、重度子痫前期组(n=30),另外选取同期于川北医学院附属医院产科住院的正常妊娠产妇30例作为健康妊娠组。取四组产妇分娩后的胎盘组织,经免疫组化、荧光定量RT-PCR检测AQP-9、HIF-1α在产妇胎盘组织中的表达,分析AQP-9、HIF-1α与妊高征产妇临床特征的相关性,以及AQP-9、HIF-1α之间的相关性。结果妊娠高血压组产妇、轻度子痫前期组产妇、重度子痫前期组产妇胎盘组织中AQP-9、HIF-1α阳性表达率,AQP-9、HIF-1αmRNA表达量均高于健康妊娠组产妇,差异均具有统计学意义(均P<0.05);轻度子痫前期组产妇、重度子痫前期组产妇胎盘组织中AQP-9、HIF-1α阳性表达率,AQP-9、HIF-1αmRNA表达量均高于妊娠高血压组产妇,差异均具有统计学意义(均P<0.05);重度子痫前期组产妇胎盘组织中AQP-9、HIF-1α阳性表达率,AQP-9、HIF-1αmRNA表达量均高于轻度子痫前期组产妇,但两组比较差异无统计学意义(P>0.05)。AQP-9、HIF-1α的表达与妊高征产妇年龄、孕产次无关,与产妇体重指数(BMI)、子痫前期家族史和高血压家族史有关。AQP-9与HIF-1α之间呈正相关(r=0.259,P=0.013)。结论AQP-9、HIF-1α在妊高征产妇的胎盘组织中为高表达,与产妇BMI、子痫前期家族史和高血压家族史有关,且AQP-9、HIF-1α之间呈正相关。
Objective To investigate the expression of aquaporin-9(AQP-9)and Hypoxia inducible factor-1α(HIF-1α)in the placental tissues of para-born women with pregnancy induced hypertension synrome(PIH),and their correlation with the clinical characteristics.Methods 90 cases of pregnant women with PIH hospitalized in Department of Obstetrics of the Affiliated Hospital of North Sichuan Medical College from January 2017 to January 2019 were enrolled,which were divided into gestational hypertension group(n=30),mild preeclampsia group(n=30),severe preeclampsia group(n=30)according to the type of symptoms.In addition,30 cases of normal pregnancy from the same hospital and same period were selected as the healthy pregnancy group.The expression of AQP-9 and HIF-1αin the placental tissues of pregnant women after delivery of four groups was detected by immunohistochemistry and fluorescence quantitative RT-PCR,and the correlation between AQP-9 and HIF-1αand the clinical characteristics of pregnant women with PIH was analyzed,as well as the correlation between AQP-9 and HIF-1α.Results The positive expression rates of AQP-9,HIF-1αand the expression quantity of AQP-9 and HIF-1αmRNA in the placental tissues of the pregnant women in the pregnancy hypertension group,the mild preeclampsia group and the severe preeclampsia group were all higher than those in the healthy pregnant group,and the differences were statistically significant(all P<0.05).The positive expression rates of AQP-9,HIF-1αand the expression quantity of AQP-9 and HIF-1αmRNA in placental tissues of women in the mild preeclampsia group and the severe preeclampsia group were all higher than those in the hypertensive pregnant women group and the differences were statistically(all P<0.05).The positive expression rates of AQP-9,HIF-1αand the expression quantity of AQP-9 and HIF-1αmRNA in the placental tissues of pregnant women in the severe preeclampsia group were all higher than those in the mild preeclampsia group,but the difference between the two groups was not statistically significant(P>0.05).The expression of AQP-9 and HIF-1αwas not correlated with age and number of pregnancies,but with body mass index(BMI),family history of preeclampsia and hypertension.AQP-9 was positively correlated with HIF-1α(R=0.259,P=0.013).Conclusions AQP-9 and HIF-1αare highly expressed in placenta of PIH,which are related to BMI index,family history of preeclampsia and hypertension.There are positive correlations between AQP-9 and HIF-1α.
