药师主导的老年慢病患者药物治疗管理研究现状

Research situation of pharmacist-guided medication therapy management in elderly patients with chronic diseases

目的了解国内外老年患者开展药物治疗管理(Medication therapy management,MTM)的研究现状,为我国老年慢性疾病和多重用药患者开展MTM提供指导。方法通过Medline、Elsevier、中国知网和万方数据库检索国内外涉及老年患者开展MTM的相关研究,对研究的类型、对象、环境、干预方式、结局指标以及研究结果进行总结和分析,并讨论我国老年患者开展MTM的现状和发展。结果目前,老年患者开展MTM的相关研究以国外文献报道为主,我国现有MTM研究多为个案报道或回顾性研究,仍缺乏高质量的前瞻性对照研究。结论多项研究已证明,对老年患者提供MTM,可以解决药物相关问题,提升治疗有效性,改善用药依从性和药物治疗安全性,以及节约医疗费用等。

Objective To learn about the current research on MTM for elderly patients at home and abroad and provide guidance for the development of MTM services for elderly patients with chronic deseases and multi-drug treatment in China.Methods We searched for the researches on MTM in the elderly patients at home and abroad in Medline,Elsevier,CNKI and WanFang databases.The type,population,setting,intervention,outcome marker and the result of the research were summarized and analyzed,and the situation and development of MTM for elderly patients were discussed.Results At present,the researches on MTM for elderly patients are mainly found in the foreign literature,and only case reports or retrospective studies are found in China,and there is still lack of high-quality prospective control studies.Conclusion Many researches have proved that MTM for elderly patients can solve DRPs,improve therapy effectiveness,promote adherence and safety,and save medical expenditure.
