右美托咪定联合丙泊酚对重症机械通气患者镇静效果和循环系统的影响

Sedative effect of dexmedetomidine combined with propofol in patients with severe mechanical ventilation and influence of circulation system

目的:研究右美托咪定联合丙泊酚对重症机械通气患者镇静效果和循环系统的影响。方法:选取2019年1月-2020年1月行机械通气的ICU患者100例,按照药物使用方案不同分为两组,各50例。对照组应用丙泊酚治疗;观察组应用右美托咪定联合丙泊酚治疗。比较两组丙泊酚用量、苏醒时间、停止输注到拔管时间、镇静前后心率(HR)、平均动脉压(MAP)及不良反应情况。结果:观察组丙泊酚用量、苏醒时间、停止输注到拔管时间及不良反应发生率均低于对照组,差异有统计学意义(P<0.05);观察组镇静后HR、MAP水平均高于对照组,差异有统计学意义(P<0.05)。结论:对重症机械通气患者采用右美托咪定联合丙泊酚的药物效果尤为显著,能够有效提高镇静效果,并减少对循环系统的影响,降低不良反应的发生。

Objective:To explore the sedative effect of dexmedetomidine combined with propofol in patients with severe mechanical ventilation and influence of circulation system.Methods:From January 2019 to January 2020,100 ICU patients undergoing mechanical ventilation were selected and divided into two groups with 50 patients in each group according to different drug use schemes.The control group was treated with propofol.The observation group was treated with dexmedetomidine combined with propofol.We compared the dosage of propofol,recovery time,time from stopping infusion to extubation,heart rate(HR),mean arterial pressure(MAP)before and after sedation and adverse reactions.Results:In the observation group,the dosage of propofol,the recovery time,the time from stopping infusion to extubation and the incidence of adverse reactions were lower than those in the control group,the difference was statistically significant(P<0.05).In the observation group,HR and MAP levels after sedation were higher than those in the control group,the difference was statistically significant(P<0.05).Conclusion:For patients with severe mechanical ventilation,dexmedetomidine combined with propofol is particularly effective,it can effectively improve the sedative effect,reduce the impact on the circulatory system and reduce the occurrence of adverse reactions.
