解剖性肝切除术对原发性肝癌的治疗效果及安全性分析

Analysis of therapeutic effect and safety of anatomical hepatectomy for primary liver cancer

目的:分析解剖性肝切除术对原发性肝癌的治疗效果和安全性。方法:2018年1-12月收治行解剖性肝切除术的原发性肝癌患者100例,作为观察组;选取同期行非解剖性肝切除术的原发性肝癌患者100例,作为对照组。比较两组手术时间、术中出血量、术后并发症发生率、术后肝功能指标水平及复发率。结果:观察组手术时间明显高于对照组,差异有统计学意义(P<0.05);观察组术中出血量和术后并发症发生率均明显低于对照组,差异有统计学意义(P<0.05);观察组术后肝功能各项指标水平均明显优于对照组,差异有统计学意义(P<0.05);观察组1年后复发率明显低于对照组,差异有统计学意义(P<0.05)。结论:解剖性肝切除术在原发性肝癌患者中应用,虽然延长了手术时间,但术中出血量少,有效降低术后并发症发生率,术后肝功能状况恢复效果好,且远期疗效高。

Objective:To explore the therapeutic effect and safety of anatomical hepatectomy for primary liver cancer.Methods:From January to December 2018,100 patients with primary liver cancer who underwent anatomical hepatectomy were selected as the observation group.100 patients with primary liver cancer who underwent non anatomical hepatectomy at the same time were selected as the control group.The operation time,intraoperative blood loss,incidence of postoperative complications,postoperative liver function index and recurrence rate were compared between the two groups.Results:The operation time of the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).In the observation group,the intraoperative blood loss and the incidence of postoperative complications were significantly lower than those in the control group,the difference was statistically significant(P<0.05).In the observation group,the postoperative liver function indexes were significantly better than those in the control group,the difference was statistically significant(P<0.05).In the observation group,the recurrence rate after one year was significantly lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:The application of anatomical hepatectomy in patients with primary liver cancer has prolonged the operation time,but the amount of intraoperative blood loss is small,it effectively reduces the incidence of postoperative complications,postoperative liver function recovery effect is good,and the long-term effect is high.
