基于中国知网数据库挖掘前列腺癌用药规律及病机治法探讨

Exploration of the Drugs Using Regularity,Pathogenesis and Treatment of Prostate Cancer Based on CNKI Database

目的:挖掘中国知网数据库(CNKI)中治疗前列腺癌的中药复方,并尝试探讨前列腺癌的用药规律及病机治法。方法:收集近10年中国知网数据库收录的治疗前列腺癌文献,按照纳入及排除标准对文献进行筛选,将纳入文献中的中药复方导入Excel 2007并建立数据库,运用中医传承辅助平台(V2.5)统计中药复方的药物及四气五味归经频次、药物组合频次并分析用药规律和提炼本病的药物核心组合及聚类新处方,最后总结并探讨前列腺癌的病机治法。结果:共收集到80个治疗前列腺癌的中药复方共有177味中药纳入研究,以补虚药、利水渗湿药、清热药与活血化瘀药为主,出现频次最高的中药是黄芪。药物四气以温寒平为主,五味以辛苦甘为主,归经以肝脾肾经为主。频次最高的药对是“白术-茯苓”频次为22次,关联规则分析显示置信度为1的有6条,并聚类获得6对核心药物组合、3首新处方。结论:本虚标实是前列腺癌发病的重要病机,扶正法贯穿始终,以“扶正-活血-解毒”为主轴,注重调理肝脾肾三经,辨证采用活血化瘀、清热利湿、通窍解毒为主线的治法,可为中医临床治疗前列腺癌提供用药参考,以通窍活血逐瘀法为主治疗前列腺癌的聚类新处方是日后值得研究的方向。

Objective:To explore the TCM compounds for prostate cancer in CNKI database and try to explore the drug using regularity and pathogenesis of prostate cancer.Methods:Literature on the treatment of prostate cancer that included in the database of CNKI in recent ten years was collected,literature was screened according to the inclusion and exclusion criteria,TCM compounds that included in the literature were inputted in the Excel 2007 to establish the database,frequency of drugs,four properties and five tastes and channel tropism as well as the drug combinations of TCM compounds were measured by Chinese medicine inheritance platform(V2.5),the drug core combination and cluster new prescription of diseases were extracted.Finally,the pathogenesis and treatment of prostate cancer were summarized and discussed.Results:A total of 80 TCM compounds for the treatment of prostate cancer were collected,and a total of 177 traditional Chinese medicine were included in the study.Tonify deficiency medicine,chearing damp and promoting diuresis drugs,heat-clearing drugs and blood-activating and stasis-eliminating compound are commonly used,among which,astragalus is most frequently used.The four properties of the medicine are mainly about warm,cold and moderate;and the five tastes are mainly about pungent,sweet and bitter.The channel tropisms are mainly about liver,spleen and kidney.Bighead atractylodes rhizome and poria cocos are used most frequently(for 22 times).Association rule analysis shows that there are 6 items with a confidence of 1.Six pairs of core drug combinations and three new prescriptions were obtained by clustering.Conclusion:Deficient root and excessive superficial is an important pathogenesis of prostate cancer.The method of strengthening the body resistance showld be adopted all the time."Strengthening the body resistance-activating blood-detoxication"showld be adopted as the principal axis.The coordination of liver,spleen and kidney showld be laid emphasis on.The treatment of activating blood circulation and removing stasis,clearing heat and removing dampness and removing poison showld be adopted.These provide reference for the clinical treatment of prostate cancer in traditional Chinese medicine.However,the new clustering prescription of treating prostate cancer with the method of promoting blood circulation and removing blood stasis is worth studying in the future.
