摘要
目的研究黏蛋白(MUC)5AC和尾型同源盒转录因子2(CDX-2)在胃癌组织中的表达及其与临床病理特征、预后的相关性。方法回顾性选择2013年1月至2013年12月于航天中心医院确诊的78例胃癌患者,收集其癌组织和癌旁正常组织。采用免疫组化染色法测定MUC5AC、CDX-2在胃癌组织及癌旁正常组织中的表达。分析MUC5AC、CDX-2表达与胃癌患者临床病理学特征及预后的关系。结果MUC5AC蛋白在胃癌组织中的阳性表达率显著低于癌旁正常组织(48.72%vs.92.31%),CDX-2蛋白在胃癌组织中的阳性表达率显著高于癌旁正常组织(52.56%vs.19.23%),差异有统计学意义(P<0.05)。低分化程度的患者胃癌组织中MUC5AC阳性表达率明显高于高分化程度者(64.29%vs.30.56%),差异有统计学意义(P<0.05)。Lauren分型小肠型、临床分期Ⅲ~Ⅳ期、有侵犯浆膜的患者胃癌组织中CDX-2阳性率分别为63.63%、69.77%、73.53%,高于弥漫型或混合型(33.33%,12.50%)、临床分期Ⅰ~Ⅱ期(31.43%)、无侵犯浆膜者(36.36%),差异有统计学意义(P<0.05)。MUC5AC阴性表达者生存时间显著低于阳性表达者(P<0.05),CDX-2阳性表达者生存时间显著低于阴性表达者(P<0.05)。结论MUC5AC在胃癌组织中呈低表达,CDX-2在胃癌组织中呈高表达,MUC5AC在胃癌中的表达与分化程度及预后呈相关性,CDX-2在胃癌中的表达与Lauren分型、临床分期、侵犯浆膜情况及预后呈相关性。
Objective To investigate the expression levels of mucins5 AC(MUC5 AC)and caudal relatedhomeodomain transcription 2(CDX-2)in gastric cancer tissues and their correlation with clinicopathological features and prognosis.Methods 78 cases of gastric cancer patients diagnosed in Aerospace Center Hospital from January 2013 to December 2013 were enrolled.The cancer tissues and adjacent normal tissues were collected for the detection of MUC5 AC and CDX-2,and their correlation with clinicopathological features and prognosis of patients with GC was also analyzed.Results The expression level of MUC5 AC protein in gastric cancer tissues was significantly lower than that in adjacent normal tissues(48.72%vs.92.31,P<0.05).The expression level of CDX-2 protein in gastric cancer tissues was significantly higher than that in adjacent normal tissues(52.56%vs.19.23%,P<0.05).The expression levels of MUC5 AC in gastric cancer tissues of patients with different degrees of differentiation were significantly different(64.29%vs.30.56%,P<0.05).The positive rates of CDX-2 in the gastric cancer tissues of patients with Lauren classification of small bowel type,clinical stageⅢtoⅣ,and serosal invasion was 63.63%,69.77%,and 73.53%,respectively,which was higher than that of diffuse or mixed type(33.33%,12.50%),clinical stageⅠtoⅡ(31.43%),non-invasive serosa(36.36%),the difference was statistically significant(P<0.05).The survival time of MUC5 AC negative expression was significantly lower than that of positive expression(P<0.05),and the survival time of CDX-2 positive expression was significantly lower than that of negative expression(P<0.05).Conclusion MUC5 AC is lowly expressed in gastric cancer tissues,which is correlated with the degree of differentiation and prognosis.CDX-2 is highly expressed in GC tissues,which is correlated to the Lauren classification,clinical stage,invasion of serosa and prognosis.
