血浆MIP-1α、Hcy、KL-6与冠状动脉粥样硬化病变程度的相关性 被引量:8

Correlation between plasma MIP-1α,Hcy,KL-6 and severity of coronary atherosclerotic lesions

摘要 目的探讨血浆单核细胞炎性蛋白-1α(MIP-1α)、血浆同型半胱氨酸(Hcy)、血清涎液化糖链抗原-6(KL-6)与冠状动脉粥样硬化病变程度的相关性。方法回顾性选取2017年1月至2019年2月福建省立医院心血管内科展开冠状动脉造影患者210例,据冠状动脉造影结果分为正常冠脉组(n=52)、单支病变组(n=54)、双支病变组(n=51)、三支病变组(n=53);采用Genisini评分法评估158例冠状动脉粥样硬化患者严重程度,分为<30分组(n=83)与≥30分组(n=75)。比较不同冠状动脉支数组的一般资料;采用酶联免疫吸附法检测不同组患者血浆MIP-1α、KL-6水平;全自动生化分析仪检测血浆Hcy水平。采用Pearason分析MIP-1α、Hcy、KL-6与冠状动脉粥样硬化病变程度的相关性,采用多因素Logistics回归分析冠状动脉粥样硬化病变的影响因素。结果与正常冠脉组对比,单支病变组、双支病变组及三支病变组总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)水平明显增高(P<0.05);而高密度脂蛋白胆固醇(HDL-C)水平显著降低(P<0.05);冠状动脉粥样硬化支数增加,TC、TG、LDL-C水平明显增高(P<0.05),而HDL-C水平显著降低,差异具有统计学意义(P<0.05);与正常冠脉组比,单支病变组、双支病变组及三支病变组血浆MIP-1α、Hcy、KL-6水平明显升高(P<0.05);冠状动脉粥样硬化支数增加,血浆MIP-1α、Hcy、KL-6水平明显升高,差异具有统计学意义(P<0.05)。<30分组血浆MIP-1α、Hcy、KL-6水平明显低于≥30分组,差异具有统计学意义(P<0.05);经Pearason分析,Gensini评分与MIP-1α、Hcy、KL-6呈正相关性(P<0.05);Gensini评分与HDL-C呈负相关性(P<0.05);经Logistic分析,LDL-C、MIP-1α、Hcy、KL-6是冠状动脉粥样硬化病变发生的独立危险因素,而HDL-C是冠状动脉粥样硬化的保护因素。结论血浆MIP-1α、Hcy、KL-6水平与冠状动脉粥样硬化发展与进程密切相关。 Objective To investigate the correlation between plasma monocyte inflammatory protein-1α(MIP-1α),homocysteine(Hcy),Krebs Von den Lungen-6(KL-6)and the severity of coronary atherosclerotic lesions.Methods A total of 210 patients undergoing coronary angiography in the Department of Cardiology,Fujian Provincial Hospital from January 2017 to February 2019 were enrolled retrospectively.According to the results of coronary angiography,they were divided into normal coronary group(n=52),single-vessel disease group(n=54),double-vessel disease group(n=51)and three-vessel disease group(n=53).Genusini score was used to evaluate the severity in 158 patients with coronary atherosclerosis,including scores<30 group(n=83)and≥30 group(n=75).General data of different coronary artery branch groups were compared.The levels of MIP-1αand KL-6 in different groups were detected by enzyme-linked immunosorbent assay,and the Hcy was detected by Automatic biochemical analyzer.The correlation between MIP-1α,Hcy,KL-6 and coronary atherosclerotic lesions was analyzed by Pearason analysis.The multivariate logistic regression was use to analyze the influencing factors of coronary atherosclerotic lesions.Results Compared with the normal coronary group,the levels of TC,TG and LDL-C in the single-vessel disease group,the double-vessel disease group and the three-vessel disease group were significantly higher(P<0.05),while the HDL-C level was significantly lower(P<0.05).The number of coronary atherosclerosis increased,and the TC,TG,LDL-C levels were increased significantly(P<0.05),while HDL-C levels decreased significantly(P<0.05).Compared with the normal coronary group,the levels of plasma MIP-1α,Hcy and KL-6 in the single-vessel disease group,double-vessel disease group and three-vessel disease group were significantly increased(P<0.05).The vessel of coronary atherosclerosis was increased and the levels of plasma MIP-1α,Hcy and KL-6 were significantly increased(P<0.05).The levels of plasma MIP-1α,Hcy and KL-6 in group<30 were significantly lower than those in≥30 group(P<0.05).Pearason analysis showed Gensini score was positively correlated with MIP-1α,Hcy and KL-6(P<0.05)and negatively correlated with HDL-C(P<0.05).Logistic analysis confirmed that the LDL-C,MIP-1α,Hcy,and KL-6 were independent risk factors for coronary atherosclerotic lesions,and that HDL-C was the protective factor for coronary atherosclerosis.Conclusion The levels of plasma MIP-1α,Hcy and KL-6 are closely related to the development and progression of coronary atherosclerosis.
