新生儿重症监护室患儿医疗操作性疼痛及其影响因素

Medical operative pain and its influencing factors in children patients in neonatal intensive care unit

目的调查新生儿重症监护室(NICU)患儿医疗操作性疼痛情况及其影响因素。方法选取2018年10月至2019年10月我院NICU收治的60例新生儿为研究对象,采用新生儿疼痛量表(NIPS)调查疼痛情况,并分析常见致痛性操作种类以及NICU新生儿医疗操作性疼痛的影响因素。结果NICU新生儿均存在不同程度的医疗操作性疼痛,新生儿疼痛量表评分(4.07±0.76)分,60例患儿住院期间共经历1210次致痛性操作,其中呼吸系统相关操作占比最高,其次为给药与标本采集;多元回归分析显示,住院时间、睡眠/清醒状态、合并肺部感染是NICU新生儿医疗操作性疼痛程度的影响因素。结论NICU新生儿发生医疗操作性疼痛较为普遍,主要发生于呼吸系统相关操作和给药与标本采集,住院时间、睡眠/清醒状态、合并肺部感染是疼痛程度的影响因素,应据此采取相应措施。

Objective To investigate the status of medical operative pain and its influencing factors in children patients in neonatal intensive care unit(NICU).Methods To select 60 cases of neonates admitted to our hospital from October 2018 to October 2019 as the study objects,and neonatal infant pain scale(NIPS)was used to investigate the pain conditions and the common types of painful operations and the influencing factors of medical operational pain in NICU neonates were analyzed.Results All NICU newborns had different degrees of medical operative pain,and the neonatal pain scale scored(4.07±0.76)points.The cases of children patients experienced 1,210 painful operations during the period of hospitalization.Among them,respiratory system related operations accounted for the highest proportion,followed by administration and specimen collection.Multiple regression analysis showed that the hospitalization time,sleep/waking state and concurrent pulmonary infection were the influencing factors affecting medical operative pain in NICU neonates.Conclusion Medical operative pain was common in neonates in NICU,which mainly occurred in respiratory system related operations,administration and specimen collection,hospitalization time,sleep/waking state and concurrent pulmonary infection were the influencing factors affecting pain degree,and corresponding measures should be taken accordingly by medical staff.
