分析老年结肠癌并肠梗阻手术治疗的方法及效果

Analysis of the method and effect of surgical treatment for elderly patients with colon cancer and intestinal obstruction

目的分析老年结肠癌并肠梗阻手术治疗的方法及效果。方法30例结肠癌并肠梗阻的老年患者,依据手术治疗方式不同将其分为对照组和研究组,各15例。对照组患者采用分期结肠切除吻合手术,研究组患者采用Ⅰ期结肠切除吻合术。比较两组患者的排气恢复时间、首次下床活动时间、住院时间、术后感染发生率以及吻合口瘘发生率。结果研究组排气恢复时间、首次下床活动时间及住院时间分别为(2.45±0.61)、(3.91±0.82)、(9.72±2.54)d,均短于对照组的(3.30±0.69)、(5.11±1.17)、(13.20±3.47)d,差异均具有统计学意义(P<0.05)。研究组术后感染发生率以及吻合口瘘发生率分别为6.67%、0,均低于对照组的40.00%、26.67%,差异均具有统计学意义(P<0.05)。结论针对结肠癌并肠梗阻的老年患者,相比于分期手术,Ⅰ期结肠切除吻合手术可在更大程度上加快患者的术后康复速度,应用价值显著,可推广。

Objective To analyze the method and effect of surgical treatment for elderly patients with colon cancer and intestinal obstruction.Methods A total of 30 elderly patients with colon cancer and intestinal obstruction were divided into control group and research group according to different surgical methods,with 15 cases in each group.Patients in the control group underwent staged colectomy and anastomosis,and patients in the research group underwent stage I colectomy and anastomosis.The exhaust recovery time,time of first offbed activity,hospitalization time,incidence of postoperative infection and incidence of anastomotic leakage were compared between the two groups.Results The exhaust recovery time,time of first off-bed activity,hospitalization time of the research group were(2.45±0.61),(3.91±0.82)and(9.72±2.54)d,which were shorter than(3.30±0.69),(5.11±1.17)and(13.20±3.47)d of the control group,and the difference was statistically significant(P<0.05).The incidence of postoperative infection and anastomotic leakage of the research group were 6.67% and 0,which were lower than 40.00% and 26.67% of the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with staged surgery,stage I colectomy and anastomosis can speed up the postoperative rehabilitation speed of elderly patients with colon cancer and intestinal obstruction to a greater extent.The treatment has significant application value and can be promoted.
