2015年济宁市部分活动性肺结核患者发现延迟影响因素Logistic回归分析

Logistic regression analysis on influencing factors of delay in discovery among some patients with active pulmonary tuberculosis,Jining city,2015

目的了解济宁市活动性肺结核患者发现延迟的影响因素,为制定肺结核患者发现政策提供科学依据。方法采用整群随机抽样方法,选取3个县为研究现场,3个县2015年登记的活动性肺结核患者为调查对象,采用非条件Logistic回归模型进行多因素分析。结果调查活动性肺结核患者249例,发现延迟患者为155例,发现延迟率为62.25%。女性(OR=0.23,P<0.01)、知晓肺结核免费政策(OR=0.16,P<0.01)、有咯血症状(OR=0.08,P<0.05)等为患者发现延迟的保护因素;农村偏远地区(OR=8.29,P<0.01)、家庭经济状况较差(OR=11.42,P<0.01)、首诊乡镇卫生院(OR=3.86,P<0.01)等为患者发现延迟的危险因素。结论济宁市活动性肺结核患者发现延迟现象严重,男性和不知晓结核病相关免费政策是肺结核患者发现延迟的危险因素。

Objective To understand the influenced factors of delay in discovery among patients with active pulmonary tuberculosis in Jining city,so as to provide scientific evidence for tuberculosis prevention and control policy formulation.Methods With cluster random sampling method,3counties were selected as the research sites,and the active pulmonary tuberculosis patients registered in 3counties in 2015were investigated.Unconditional logistic regression model was used for multivariate analysis.Results A total of 249pulmonary tuberculosis patients was included in the study,with155delayed discovery cases,the delay rate was 62.25%.Women(OR=0.23,P<0.01),the awareness of the free TB policy(OR=0.16,P<0.01)and hemoptysis(OR=0.08,P<0.05)were protective factors for delayed discovery patients with TB.Living in rural and remote areas(OR=8.29,P<0.01),lower income(OR=11.42,P<0.01),the first visit in the town hospital(OR=3.86,P<0.01)were the risk factors for the delay in discovery.Conclusion Delay in discovery of patients with active pulmonary tuberculosis in Jining city is serious,the male and the unacquaintance of the free TB policy are the risk factors for the delay in discovery of patients with TB.
