培美曲塞联合铂类药物三线及以上治疗晚期乳腺癌的效果观察

Efficacy of pemetrexed plus platinum-based drugs as third-line or above treatment of advanced breast cancer

目的探讨培美曲塞联合铂类药物三线及以上治疗晚期乳腺癌的临床效果。方法选取2016年1月至2019年1月间空军军医大学西京医院收治的80例晚期乳腺癌患者,采用随机数字表法分为观察组和对照组,每组40例。对照组患者采用培美曲塞单药治疗,观察组患者采用培美曲塞联合顺铂治疗,比较两组患者的治疗效果、不良反应及生活质量。结果观察组患者疾病控制率为70.0%,高于对照组的45.0%,差异有统计学意义(P<0.05)。观察组患者骨髓抑制、恶心呕吐、中性粒细胞减少、厌食和乏力等不良反应发生率均高于对照组,差异均有统计学意义(均P<0.05)。治疗前,两组患者体力状况评分标准(PS)和生活质量量表(QOL)评分比较,差异无统计学意义(P>0.05);治疗后,两组患者PS和QOL评分均升高,且观察组均高于对照组,差异均有统计学意义(均P<0.05)。结论培美曲塞联合铂类药物三线及以上治疗晚期乳腺癌效果较好,可改善患者生活质量,虽不良反应较多,但患者可耐受。

Objective To investigate the efficacy of pemetrexed plus platinum-based drugs as third-line or above treatment of advanced breast cancer.Methods A total of 80 patients with advanced breast cancer admitted to Air force Military Medical University Xijing Hospital from January 2016 to January 2019 were selected as the study subjects.They were divided into two groups using random number table method with 40 patients each.The observation group was treated with pemetrexed plus platinum-based drugs and the control group was treated with pemetrexil monotherapy.The treatment efficacy,adverse reactions and quality of life were compared between the two groups.Results The disease control rate was 70%in the observation group,which was significantly higher than 45%of the control group(P<0.05).The incidence of adverse reactions including myelosuppression,nausea and vomiting,neutropenia,anorexia and fatigue was significantly higher in the observation group than in the control group(all P<0.05).There was no significant difference in the PS and QOL scores between the two groups before treatment(P>0.05).In contrast,performance scale(PS)and quality of life(QOL)scores increased in both groups after treatment,and the scores were significantly higher in the observation group than in the control group(all P<0.05).Conclusion Pemetrexed combined with platinum-based drugs shows high efficiency as third-line or above treatment of advanced breast cancer.It significantly improves quality of life with good tolerance despite many adverse reactions.
