疼痛护理结合PDCA循环管理对骨转移癌放疗患者疼痛及希望水平的影响

Effects of pain nursing combined with PDCA circulation management on pain and hope level in patients with bone metastases on radiotherapy

目的探讨疼痛护理结合计划-执行-检查-处理(PDCA)循环管理对骨转移癌放疗患者疼痛和希望水平的影响。方法选取2017年1月至2019年12月间营口市中心医院收治的78例骨转移癌放疗患者,采用简单随机分组法分为研究组和对照组,每组39例。对照组患者采用常规护理干预,研究组患者采用疼痛护理结合PDCA循环管理护理干预,比较两组患者术后6h、24h、48h和72h的视觉模拟评分(VAS)评分,以及干预前后的希望水平评分。结果两组患者VAS评分,术后72h<术后48h<术后24h<术后6h,且研究组患者VAS评分在术后48h和72h均较对照组低,差异均有统计学意义(均P<0.05)。干预前,两组患者对现实和未来的积极态度、采取积极行动、与他人保持亲密关系和总希望水平评分比较,差异无统计学意义(P>0.05)。干预后,两组患者希望水平各项评分均升高,且研究组高于对照组,差异均有统计学意义(均P<0.05)。结论骨转移癌患者采用疼痛护理结合PDCA循环管理护理干预,可有效提高患者的希望水平,减轻疼痛。

Objective To investigate the effects of pain nursing combined with Plan-Do-Check-Act(PDCA)circulation management on the pain and hope level in patients with bone metastasis on radiotherapy.Methods Seventy patients with metastatic bone tumors who received radiotherapy at Yingkou Central Hospital from January 2017 to December 2019 were selected as study subjects.The patients were divided into an experimental group and a control group,with 39 patients each.The experimental group was given pain intervention combined with PDCA circulation management nursing intervention and the control group was given conventional nursing intervention.Visual analogue scores at 6 h,24 h,48 h,and 72 h after the surgery and hope level scores before and after intervention were compared between the two groups.Results Visual analogue scale(VAS)scores were lower at 48 h than 72 h followed by 24 h and 6 h after the surgery in both groups and VAS scores were lower in the experimental group than in the control group at 48 h and 72 h after the surgery(all P<0.05).Before intervention,there was no significant difference in the positive attitudes to reality and the future,positive actions,close relationship with others,and the overall hope level scores between the two groups(P>0.05).After the intervention,the hope level scores significantly increased in both groups with the experimental group higher than the control group(all P<0.05).Conclusion Pain nursing combined with PDCA circulation management nursing intervention can effectively improve the hope level and reduce pain in patients with metastatic bone cancer.
