基于预后营养指数的营养干预在肺癌手术患者中的应用

Application of nutritional intervention based on prognostic nutritional index in patients with lung cancer surgery

目的探讨基于预后营养指数的营养干预在肺癌手术患者中的应用效果。方法采用便利抽样法,选取2017年4月—2019年4月湖州市第一人民医院收治的56例肺癌患者作为研究对象,按照随机数字表法分为观察组与对照组,每组各28例。观察组行基于预后营养指数的营养干预,对照组行常规营养干预。比较两组患者的胸腔闭式引流管拔除时间、住院时间、并发症发生情况、营养状态及生活质量。结果观察组患者的胸腔闭式引流管拔除时间、住院时间均短于对照组,并发症例数少于对照组,差异均有统计学意义(P<0.05)。术后1、3周,观察组预后营养指数均高于对照组,差异有统计学意义(P<0.05)。术后3个月,观察组患者的躯体、情绪及整体生活质量评分高于对照组,疲乏、恶心呕吐、食欲减退、便秘、腹泻、气促评分低于对照组,差异均有统计学意义(P<0.05)。结论基于预后营养指数的营养干预可缩短肺癌患者术后恢复时间,降低并发症发生率,提高躯体功能及整体生活质量。

Objective To explore the application effect of nutritional intervention based on prognostic nutritional index in patients with lung cancer surgery.Methods Using the convenient sampling method,56 patients with lung cancer who were admitted to Huzhou First People's Hospital from April 2017 to April 2019 were selected as the research objects.According to the random number table method,they were divided into the observation group and the control group,with 28 cases in each group.The observation group received nutritional intervention based on the prognostic nutritional index,and the control group received conventional nutritional intervention.The removal of thoracic closed drainage tube,length of hospital stay,complications,nutritional status and quality of life of patients were compared between the two groups.Results The removal time of the closed thoracic drainage tube and length of hospital stay in the observation group were shorter than those in the control group,and the number of cases of complications was less than that that in the control group,and the differences were statistically significant(P<0.05).At 1 and 3 weeks after surgery,the prognostic nutritional index of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).At three months after the operation,the scores of body,mood and overall quality of life in the observation group were higher than those in the control group,while the scores of fatigue,nausea and vomiting,poor appetite,constipation,diarrhea and shortness of breath were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusions Nutritional intervention based on prognostic nutritional index can shorten the postoperative recovery time,reduce the incidence of complication and improve the physical function and overall quality of life of patients with lung cancer.
