肿瘤坏死因子-α、内毒素与重症胸部损伤后凝血功能紊乱发生机制的关系

The Relationship Between Tumor Necrosis Factor-endotoxin and the Mechanism of Coagulation Dysfunction After Severe Chest Injury

摘要 目的探讨肿瘤坏死因子-α(TNF-α)、内毒素(LPS)与重症胸部损伤后凝血功能紊乱发生机制的关系。方法收集2017年3月至2019年12月在呼和浩特市第一医院胸外科住院的创伤指数TI≥17分(除外合并颅脑、腹部损伤及死亡的)胸部损伤患者68例,在就诊及住院后予以积极抢救、治疗的同时抽血化验部分活化凝血酶原时间APTT、凝血酶时间TT、肿瘤坏死因子TNF-α和内毒素LPS,对检验结果进行相关性分析检验。结果 APTT与TNF-α:r=0.8635,P<0.001;APTT与LPS:r=0.8736,P<0.001;TT与TNF-α:r=0.8633,P<0.001;TT与LPS:r=0.8274,P<0.001。结论重症胸部损伤后凝血功能障碍的发生与TNF-α和LPS的高表达存在着显著的相关性,表明TNF-α和LPS参与了重症胸部损伤凝血功能紊乱发生的过程。 Objective To investigate the relationship between TNF-endotoxin(LPS)and the mechanism of coagulation disorder after severe chest injury.Methods Collection in March 2017 to December 2019 in Hohhot thoracic surgery first hospital trauma index TI 17 points or more(with the exception of merger of brain, abdominal injury and death), 68 cases of chest injury patients, hospitalized in clinic and at the same time actively rescue and treatment after exsanguinate assay of activated APTT prothrombin time,thrombin time TT,tumor necrosis factor TNF-α and endotoxin LPS,correlation analysis was carried out on the test results.Results APTT and TNF-α:r=0.8635,P<0.001;APTT and LPS:r=0.8736,P<0.001;TT and TNF-α:r=0.8633,P<0.001;TT and LPS:r=0.8274,P<0.001.Conclusion The occurrence of coagulation dysfunction after severe chest injury is significantly correlated with the high expression of TNF-αand LPS, indicating that TNF-and LPS are involved in the occurrence of coagulation dysfunction in severe chest injury.
