摘要 目的建立基于RBRVS的护理绩效薪酬分配方案,为护理绩效考评提供科学的决策依据。方法2015年1月1日—11月30日护理绩效考核组对全院医护的执行项目进行总体筛选评价,确定计入绩效核算的服务项目,组建绩效考核项目工时测量组,根据开展诊疗项目的技术含量、人力成本、风险程度、劳动强度及占历史医疗项目比重等多种因素,通过比较医疗服务中投入的各类资源要素成本高低来计算每次服务的相对值,实行院科个人三级分配制。自行设计调查表,2015年11月对全院256名护理人员进行满意度问卷调查。结果确定护理项目172类并分别赋予权重系数;通过调查,护士对认同绩效分配体系体现优劳优酬绩效管理的满意度实施前后无差异性,其他条目的满意度均有提升,差异具有统计学意义(P<0.01)。结论科学的RBRVS的护理绩效分配体系可以横向、纵向地进行医院、科室、个人之间的护理业绩评价,为临床护理绩效考评提供科学的决策依据。 Objective To establish a compensation allocation scheme for nursing performance based on RBRVS,and to provide scientific decisionmaking basis for nursing performance evaluation.Methods From January 1,2015 to November 30,2015,the nursing performance assessment team conducted a general screening and evaluation of the implementation projects of the hospital's doctors and nurses,and determined the service items included in the performance accounting.Establish a performance appraisal project time measurement group mainly composed of nursing postgraduates.According to the technical content,human cost,risk degree,labor intensity and proportion of historical medical projects,the relative value of each service is calculated by comparing the cost of various resource elements invested in medical services.Value,the three-level distribution system for individual departments of academies and departments shall be implemented.A self-designed questionnaire was used to investigate the satisfaction of 256 nurses in the hospital in November 2015.Results The 172 categories of nursing items were determined and weighted coefficients were given respectively.According to the survey,there was no difference in the satisfaction of nurses before and after the implementation of the recognition performance distribution system reflecting the performance management of excellent pay,while the satisfaction of other items was improved,with statistical significance(P<0.01).Conclusion Scientific RBRVS nursing performance allocation system can evaluate nursing performance horizontally and vertically among hospitals,departments and individuals,provide scientific decision-making basis for clinical nursing performance appraisal.
