全身麻醉联合硬膜外麻醉在妇科腹腔镜手术患者中的应用价值

Application Value of General Anesthesia Combined with Epidural Anesthesia in Gynecological Laparoscopic Patients

目的探讨全身麻醉联合硬膜外麻醉在妇科腹腔镜手术患者中的应用价值。方法选取2017年10月至2018年10月于沈阳市妇婴医院妇科接受腹腔镜手术的62例患者,按照患者的入院先后顺序将其分为对照组和观察组,每组31例。对照组患者单纯实施全身麻醉,观察组患者在对照组基础上加以硬膜外麻醉,观察两组患者的临床指标改善情况和麻醉后患者子宫的松弛改善情况,并比较两组的临床效果。结果观察组心率低于对照组,平均动脉压高于对照组,麻醉苏醒时间、导管拔出时间、手术时间以及自主呼吸恢复时间均短于对照组,差异均有统计学意义(P<0.05);观察组患者麻醉后子宫松弛优良率高于对照组,差异有统计学意义(P<0.05)。结论针对接受妇科腹腔镜手术治疗的患者采用全身麻醉联合硬膜外麻醉的效果明显优于全身麻醉,是理想的麻醉方案。

Objective To investigate the clinical application value of general anesthesia combined with epidural anesthesia in gynecological laparoscopic patients.Methods From October 2017 to October 2018,62 patients who underwent laparoscopic surgery in Department of Anesthesiology,Shenyang Maternity and Infant Hospital were selected and divided into the control group and the observation group according to the admission order of the patients,31 cases in each group.The patients in the control group were only given general anesthesia,and the patients in the observation group were given epidural anesthesia on this basis.The improvement of clinical indicators and uterine relaxation after anesthesia in the two groups were observed,and the clinical application effects were compared.Results The heart rate of the observation group was lower than that of the control group,and the average arterial pressure was higher than that of the control group,the recovery time from anesthesia,catheter removal time,operation time and spontaneous breathing recovery time were shorter than those of the control group,and the differences were statistically significant(P<0.05).The excellent rate of uterine relaxation after anesthesia in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion For gynecologic laparoscopic patients,the clinical effect of general anesthesia combined with epidural anesthesia is significantly better than general anesthesia alone.It is an ideal anesthesia scheme.
