替米沙坦联合血液透析治疗尿毒症合并高血压患者的临床疗效

Clinical Efficacy of Telmisartan Combined with Hemodialysis in the Treatment of Patients with Uremia Complicated withHypertension

目的观察并评价对尿毒症合并高血压患者在血液透析基础上联合替米沙坦方案加以干预所取得的效果。方法本文所选择的研究对象入院时间为2017年11月至2018年10月,均为我院收治的78例尿毒症合并高血压患者,采用随机方法将其分为观察组与对照组,每组39例。对照组常规维持性血液透析治疗,观察组在对照组治疗基础上,给药替米沙坦治疗,为患者进行为期12周的治疗。对两组患者治疗后的血压状况和肾功能指标改善情况进行评价和比较。结果治疗后,观察组的收缩压、舒张压均低于对照组,差异存在统计学意义(P<0.05);治疗后两组患者的血尿素氮、血肌酐、eGFR、血尿酸、尿微量白蛋白以及尿β2微球蛋白相关水平进行比较,观察组明显优于对照组,存在统计学差异性(P<0.05);两组患者不良反应发生率差异无统计学意义(P>0.05)。结论尿毒症合并高血压患者在血液透析基础上联合替米沙坦进行治疗,能帮助患者维持良好的血压水平,改善患者的肾功能指标,并且具有较好的安全性,值得推广。

Objective To observe and evaluate the effect of intervention with telmisartan on the basis of hemodialysis in patients with uremia and hypertension.Methods The subjects selected in this study were from November 2017 to October 2018.All of the 78 patients with uremia and hypertension who were admitted to our hospital during this period were randomly divided into observation group and control group,the average of each group was 39 cases.The control group was treated with conventional maintenance hemodialysis during the treatment.The observation group was treated with the control group as the basis of the treatment.At the same time,the patient was administered telmisartan for the patient,and the treatment was performed for 12 weeks.The blood pressure status and the improvement of the patient's renal function index were evaluated and compared.Results After treatment,the systolic blood pressure and diastolic blood pressure of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05);blood urea nitrogen,blood creatinine,eGFR,blood uric acid,and urine microalbumin and compared with the relative levels of urineβ2 microglobulin of the two groups after treatment,the observation group was significantly better than the control group,there was a statistical difference(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The treatment of patients with uremia and hypertension combined with telmisartan on the basis of hemodialysis can help patients maintain a good blood pressure level,improve the patients'renal function indexes,and has good safety and is worthy of promotion.
