云南省边境县涉外婚姻家庭HIV感染现状

Size of mixed marriages and the HIV infection status in the border counties of Yunnan province

目的了解云南省边境县涉外婚姻人群规模,掌握该人群艾滋病病毒(HIV)感染状况。方法2014-2018年,以村为单位,采用统一下发的数据库收集辖区内所有涉外婚姻家庭的规模数据及人口学信息,逐级汇总上报,并对其开展HIV动员检测。结果全省涉外婚姻家庭规模有49903户,德宏州、保山市、文山州和普洱市占75.3%。外籍家庭成员中缅甸占75.7%,女性占96.5%,20~39岁占73.2%。对84113人开展了HIV检测,HIV抗体阳性794人,阳性率0.9%。不同县区该人群的HIV阳性率在0.1%~3.4%之间,不同年龄、性别和民族的中国籍家庭成员HIV阳性率差异有统计学意义(χ^2=144.2,P<0.001;χ^2=25.14,P<0.001;χ^2=231.74,P<0.001)。外籍家庭成员中,不同国籍、年龄、性别和结婚时长其HIV阳性率差异有统计学意义(χ^2=34.61,P<0.001;χ^2=120.48,P<0.001;χ^2=7.01,P=0.01;χ^2=18.21,P<0.001)。结论云南省边境县涉外婚姻家庭规模大,HIV感染率高,不同地区及人口学特征HIV感染率差异较大。建议将该人群纳入边境地区艾滋病防治常规工作。

Objective To understand the size of mixed marriages in the border counties of Yunnan province and the HIV infection status of the population.Methods From 2014 to 2018,the size data and demographic information of all mixed marriages were collected through a unified database summarized and reported based on the villages as the unit,and HIV testing was conducted accordingly.Results There were 49903 mixed marriages and transnational families in Yunnan,accounting for 75.3%in Dehong,Baoshan,Wenshan and Pu’er cities.Out of the family members,Myanmar accounted for 75.7%,with 96.5%female,and 73.2%between 20-39 years old.Among 84113 people receiving HIV testing,794 were HIV-positive,with the rate of 0.94%,and between 0.1%and 3.4%in different counties and districts.There were statistically significant differences in HIV positive rates among Chinese family members of different ages,genders and nationalities(χ~2=144.2,P<0.001;χ~2=25.14,P<0.001;χ~2=231.74,P<0.001).There were statistically significant differences in HIV positive rates among transnational family members of different nationalities,ages,genders and marriage duration(χ~2=34.61,P<0.001;χ~2=120.48,P<0.001;χ~2=7.01,P=0.01;χ~2=18.21,P<0.001).Conclusion There are a large number of mixed marriages and transnational families in the border counties of Yunnan,with s high rate of HIV infection.The HIV infection varies greatly in different regions and demographic characteristics.It is strongly suggested to strengthen AIDS prevention and control among the group in the border areas.
