武汉市梅毒规范化医疗服务试点模式评价

Evaluation of the pilot model of standardized medical services for syphilis infection in Wuhan

目的规范武汉市梅毒管理医疗服务,提高梅毒疫情报告准确性,控制梅毒传染源,降低梅毒发病率。方法评价建立管理模式、制定方案与流程、规范管理梅毒医疗服务以及考核医疗机构的效果。结果医疗机构梅毒医疗服务能力综合评分≥85分的比例从2015年的77.1%上升到2017年的91.1%;一期和二期梅毒发病率从2015年的9.93/10万逐年降至2018年的4.48/10万,胎传梅毒发病率从2015年的38.98/10万逐年降至2018年的8.99/10万;报告梅毒的诊断准确率从2015年的91.5%逐年上升至2018年的96.3%;梅毒转介数从2015年的524人逐年增加至2018年的3875人。结论开展梅毒规范化医疗服务工作,可以提高梅毒疫情准确性,对检测发现的梅毒感染者进行有效转介和规范治疗,能控制梅毒传染源,降低梅毒发病率,值得进一步推广。

Objective To standardize syphilis medical services in Wuhan city,to improve the accuracy of syphilis epidemic report,control the source of syphilis infection and reduce the incidence.Methods Evaluation was conducted to know the effect of establishment of management model,working plans and procedures,standardized management of syphilis medical services and assessment of medical institutions.Results The comprehensive score of syphilis medical service ability of medical institutions increased from 77.1%in 2015 to 91.1%in 2017.The incidence of stage I and II syphilis decreased from 9.93/100000 in 2015 to 4.48/100000 in 2018,and the incidence of fetal syphilis decreased from 38.98/100000 in 2015 to 8.99/100000 in 2018.The diagnostic accuracy of syphilis reported increased from 91.46%in 2015 to 96.32%in 2018.The number of syphilis referrals increased from 524 in 2015 to 3875 in 2018.Conclusion The standardized medical services for syphilis infection can improve the accuracy of syphilis epidemic situation,effective referral and the treatment,and reduce the incidence of syphilis,which is worthy of further promotion.
