腹部推拿按摩对产后子宫复旧疗效的观察

Observation on the therapeutic effect of abdominal massage on postpartum uterine rejuvenation

目的研究腹部推拿按摩对产后子宫复旧的疗效。方法选择2016年9月~2019年3月我院收治的200例顺产产妇,根据抽签法将产妇随机分为A、B两组,每组各100例。A组采用腹部推拿按摩的治疗方法,B组不做特殊处理,使其自然康复。观察至产后42 d,比较两组产妇的产后宫底下降情况、排尿时间、肛门排气时间及恶露干净时间等指标,并比较两组产妇生产1周后的子宫总复旧率。结果A组产妇产后平均宫底高度为(10.07±1.34)cm,低于B组的(16.37±2.42)cm,差异有统计学意义(P<0.05)。A组产妇产后排尿时间(8.65±1.13)h、肛门排气时间(30.16±5.16)h,分别短于B组的(15.46±1.44)h、(37.90±7.45)h,差异有统计学意义(P<0.05)。A组产妇产后恶露干净时间为(12.67±1.64)d,短于B组的(21.57±2.30)d,差异有统计学意义(P<0.05);A组产妇产后子宫总复旧率为99.00%,高于B组的85.00%,差异有统计学意义(P<0.05)。结论产后对产妇采取腹部推拿按摩治疗,能有效促进子宫收缩、及时排出恶露、防止组织残留,同时可缩短排尿及肛门排气时间、防止尿潴留、减少产后出血,有利于子宫复旧,具有较高的临床价值。

Objective To study the effect of abdominal massage on postpartum uterine rejuvenation.Methods A total of 200 cases of natural delivery women admitted to the hospital from September 2016 to March 2019 were randomly divided into group A and group B according to the lottery method,with 100 cases in each group.Group A adopted abdominal massage therapy,and group B didn't undergo special treatment to allow it to recover naturally.The patients were observed to postpartum 42 d.The postpartum uterine fundus decline,urination time and anal exhaust time,lochia clean time between the two groups were compared.The uterine recovery rate between the two groups after one week of delivery was compared.Results The average postpartum uterine height of group A was(10.07±1.34)cm,which was lower than that(16.37±2.42)cm of group B,and the difference was statistically significant(P<0.05).The postpartum urination time(8.65±1.13)h and anal exhaust time(30.16±5.16)h in group A were shorter than(15.46±1.44)h and(37.90±7.45)h in group B,respectively,and the difference was statistically significant(P<0.05).The cleansing time of lochia in group A was(12.67±1.64)d,which was shorter than(21.57±2.30)d in group B,and the difference was statistically significant(P<0.05).The total postpartum uterine recovery rate in group A was 99.00%,higher than that in group B(85.00%),and the difference was statistically significant(P<0.05).Conclusion Abdominal massage treatment for postpartum women can effectively promote uterine surgery,discharge the lochia in time,prevent tissue residue.And it can shorten the time of urination and anal exhaust,prevent urinary retention,reduce postpartum hemorrhage,conducive to uterine contraction,improve uterine recovery significantly,which has high clinical value.
