利用二维斑点追踪成像技术评价化疗药物对乳腺癌患者左心功能损害的价值研究 被引量:5

摘要 目的探讨二维斑点追踪成像技术评价化疗药物对乳腺癌患者左心功能损害的价值。方法选取本院2018年1月至2020年1月收治的50例乳腺癌术后患者纳入本次研究,所有患者均进行术后辅助化疗,采用以蒽环类药物为主的化疗方案。所有患者均不联合使用其他化疗药物,均进行6个周期,每21 d为一个周期。根据患者化疗周期进行统计,纵向对比患者心功能的变化情况。结果(1)治疗前后常规超声心动图测值及常规指标比较差异均无统计学意义(均P>0.05);(2)治疗后不同切面的心内膜下心肌平均应变峰值均明显低于治疗前,差异均有统计学意义(均P<0.05)。结论二维斑点追踪成像技术可评价化疗前后左心功能及化疗药物对各层心肌的浸润程度,为临床妥善利用化疗药物降低对乳腺癌患者左心功能损害提供依据。 Objective To evaluate the value of two-dimensional speckle tracking imaging in the evaluation of left ventricular dysfunction caused by chemotherapy drugs in breast cancer patients.Methods 50 patients with breast cancer treated in our hospital from January 2018 to January 2020 were included in this study.All patients were treated with adjuvant chemotherapy after operation,and anthracycline-based chemotherapy was adopted,not combined with other chemotherapy drugs,21 days one cycle for 6 cycles.According to the chemotherapy cycle of patients,the changes of cardiac function of patients were compared longitudinally.Results 1)There were no statistically significant differences in conventional echocardiography and conventional indexes before and after treatment(all P>0.05).2)After treatment,the average peak strain of subendocardial myocardium in different sections were significantly lower than those before treatment(all P<0.05).Conclusion Two-dimensional speckle tracking imaging can evaluate the left ventricular function before and after chemotherapy and the infiltration degree of chemotherapy drugs into each layer of myocardium,which provides evidences for clinical proper use of chemotherapy drugs to reduce left ventricular dysfunction in breast cancer patients.
