IgG4相关自身免疫性胰腺炎的诊治:5例报道并文献复习

Diagnosis and treatment of IgG4-related autoimmune pancreatitis:5 cases report and literature review

目的探讨IgG4相关自身免疫性胰腺炎(IgG4-related autoimmune pancreatitis,IgG4-AIP)的诊治。方法回顾性分析我院2019年1月1日至2019年12月1日收治的经病理确诊为IgG4-AIP的5例患者临床资料,并复习相关文献。结果5例患者主要临床症状为腹痛、黄疸、体质量减轻,CT主要表现为胰头肿大或胰腺弥漫性肿大,5例患者均有IgG4升高,病理均见大量IgG4阳性淋巴细胞浸润,糖皮质激素治疗有效。结论IgG4-AIP临床症状多样,易误诊,临床医师应提高警惕,糖皮质激素治疗有效。

Objective To investigate the diagnosis and treatment of IgG4-related autoimmune pancreatitis(IgG4-AIP).Methods The clinical data of 5 cases of IgG4-AIP diagnosed by pathology in our hospital from Jan.1,2019 to Dec.1,2019 were retrospectively analyzed,and the literatures were reviewed.Results The main clinical symptoms of the 5 patients were abdominal pain,jaundice and weight loss,and the main manifestations of CT were enlargement of the head of the pancreas or diffuse enlargement of the pancreas.IgG4 was increased in all the 5 patients,and a large number of IgG4-positive lymphocyte infiltration was observed pathologically.Glucocorticoid therapy was effective.Conclusion The clinical symptoms of IgG4-AIP are diverse and easy to be misdiagnosed.Clinicians should be vigilant and glucocorticoid therapy is effective.
