老年综合评估及高血压管理在住养高血压老人中的应用研究

Application of comprehensive geriatric assessment and hypertension management in the elderly patients with hypertension

目的探讨老年综合评估(CGA)及高血压管理在住养高血压老人中的应用。方法将入住江苏省老年公寓的150例高血压病人分为管理组和对照组,每组75例。对照组进行一般门诊随访、药物调整和普通生活指导,管理组进行为期1年的综合评估和干预,对2组血压,MMSE、抑郁自评量表(PHQ-9)、焦虑自评量表(GAD-7)和Barthel指数(BI)评分进行比较分析。结果干预后,管理组SBP和DBP均明显低于干预前(P<0.01),与对照组比较,血压差异亦有统计学意义(P<0.01)。干预12个月,管理组的MMSE、BI评分明显升高,PHQ-9及GAD-7评分明显下降(P<0.01),2组间比较,差异均有统计学意义(P<0.01)。结论对患有高血压的住养老人实施CGA,通过积极地干预及合理的药物治疗,不但可以有效地控制血压,同时还可以提高住养老人的生活质量。

Objective To explore the application of comprehensive geriatric assessment(CGA)and hypertension management in the elderly patients with hypertension.Methods A total of 150 hypertension patients were enrolled and divided into management group and control group,with 75 cases in each group.The control group was given routine following-up,drug adjustment and life guidance.The management group were conducted comprehensive evaluation and intervention.The level of blood pressure,the scores of Mini-mental State Examination(MMSE),Patien Health Questionnare(PHQ-9),Generalized Anxiety Disorder(GAD-7)and Barthel Index(BI)in the two groups were observed and analyzed.Results After the intervention,the levels of systolic blood pressure(SBP)and diastolic blood pressure(DBP)in the management group were effectively controlled,and significantly lower than those in the control group.The scores of MMSE and BI were significantly increased,and the scores of PHQ-9 and GAD-7 were significantly decreased in the management group,and there were significant differences between the management group and control group 12 months after the intervention(P<0.05).Conclusions Appling the CGA,active intervention and reasonable drug treatment not only can effectively control blood pressure,but also improve the quality of life in the elderly with hypertension.
