腓骨截骨术结合关节镜下有限清理术对早期膝关节骨性关节炎患者术后膝关节功能的影响

Effect of fibular osteotomy combined with arthroscopic limited debridement on postoperative knee joint function in patients with early knee osteoarthritis

目的探讨腓骨截骨术结合关节镜下有限清理术治疗早期膝关节骨性关节炎的效果。方法回顾性分析2015年1月至2020年1月于本院就诊的120例早期膝关节骨性关节炎患者的临床资料。依据手术方法将患者分为单独组(n=60)和结合组(n=60)。单独组仅给予关节镜下有限清理术,结合组给予腓骨截骨术结合关节镜下有限清理术。比较两组的手术相关指标、疼痛程度、膝关节功能及术后并发症发生情况。结果结合组的手术时间显著长于单独组,差异具有统计学意义(P<0.05);两组的术中出血量及住院时间比较,差异无统计学意义(P>0.05)。手术后,两组的静息时及活动时VAS评分均较手术前显著降低,Lysholm及HSS评分均较手术前显著升高,且结合组优于单独组,差异具有统计学意义(P<0.05)。两组的术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论采用腓骨截骨术结合关节镜下有限清理术治疗早期膝关节骨性关节炎,可以显著缓解疼痛,改善患者的膝关节功能,值得在临床应用推广。

Objective To investigate the effect of fibular osteotomy combined with arthroscopic limited debridement in the treatment of early knee osteoarthritis.Methods The clinical data of 120 patients with early knee osteoarthritis treated in our hospital from January 2015 to January 2020 were retrospectively analyzed.According to the operation method,the patients were divided into single group(n=60)and combined group(n=60).The single group was only given arthroscopic limited debridement,while the combined group was given fibular osteotomy combined with arthroscopic limited debridement.The operation related indexes,pain degree,knee joint function and postoperative complications were compared between the two groups.Results The operation time of the combined group was significantly longer than that of the single group,and the difference was statistically significant(P<0.05);there were no significant differences in intraoperative blood loss volume and hospital stay between the two groups(P>0.05).After operation,the VAS scores at rest and activity in the two groups were significantly lower than those before operation,while the scores of Lysholm and HSS were significantly higher than those before operation,and those of the combination group were better than the single group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of postoperative complications between the two groups(P>0.05).Conclusion Fibular osteotomy combined with arthroscopic limited debridement in the treatment of early knee osteoarthritis can significantly relieve pain and improve the knee joint function of patients,which is worthy of clinical application and promotion.
